RE: OLAMIDE MATHEW AGBAJE/ MALE/ 29YEARS
This is to confirm that the above -named Prospective Corps Member is a registered
outpatient at our facility and is currently receiving medical management for Migraine with aura
since September 2025 .
He has been under continuous management due to recurrent episodes of headaches, diagnose as
migraines , requiring regular medication and strict dietary control.

Migraine is a neurological condition characterized by recurring episode of severe headache, often
accompanied by other Pulsating pain, Sensitivity to light and sound, Nausea and vomiting
Dizziness which are prevalent in the NYSC camp environment. The patient is currently on the following
treatments:
· Ergotamines (20mg daily)
· Triptans – for symptom relief by constricting blood vessels
· Dietary modifications – strict avoidance of spicy, fried, and acidic foods
· Pain relievers – to prevent symptom flare ups and complications
His last consultation was on Aug 15, 2025, with a scheduled follow-up on Sept 29, 2025. Given the high
physical demands, irregular mealtimes, and limited dietary options present in the NYSC orientation camp,
he is strongly advised to exeat camp to prevent serious health complications such as Pulsating or
throbbing pain .
