ADMINISTRATIVE POLICY

Medical leave

1. Objective:
Standardize medical leaves in accordance with current regulations, which allows for the
the company for the collection of medical subsidies from Essalud.

2. Scope:
This procedure applies to all employees of the company GECODEM.

3. Related documents:
3.1 Article 42 of the Internal Work Regulations.

3.2 Directive No. 006-GGG-Essalud 2009

4. Responsibility:
4.1 The Human Resources area must ensure compliance with the procedure.

4.2 The heads are responsible for enforcing what is established in each of their areas.

4.3 The Human Resources area is responsible for receiving, registering, and safeguarding the
documents related to medical leaves submitted by each worker.

5. Definitions:
5.1 MEDICAL REST

It is the period of temporary incapacity to work, authorized by the treating physician and
granted to the worker, so that he may keep the necessary rest or break to achieve a
physical and mental recovery.
6. Procedure:

Any employee who presents a work absence due to health reasons


you must present the original medical leave to the Human Resources department with
the corresponding sustenance.
The presentation of such a document can even be through a family member, within
the seventy-two (72) hours after it has been issued or send the complete documentation

6.1 MEDICAL REST DOCUMENT

Medical leaves can be granted by:

6.1.1 Essalud doctors (in Essalud Clinics and/or Hospitals), through the Certificate
of Temporary Work Disability - CITT, which is a form used by
Essalud.

6.1.2 Private doctors (in Private Clinics, Hospital of Solidarity, Hospitals


or Posts of the Ministry of Health and others), through a medical certificate
specific member or valued species of the Medical College of Peru, issued
by general physician or specialist physician with RNE (with valid registration in the College)
Doctor from Peru), and must indicate:
• The full name of the collaborator, age, and ID number.
• Diagnosis that must correspond to the specialty of the doctor.
• Medical leave period specifying start and end date or the total of
days from the date of issue.
• Legible signature and seal (the signature must be the same as registered on your ID).

6.2 DOCUMENTS TO BE ATTACHED TO THE MEDICAL LEAVE

6.2.1 Medical leaves issued by Essalud,

6.2.1.1 Outpatient care:


i. Medical leave issued in Essalud format: Certificate of Incapacity for
Temporary Work (CITT).
ii. Copy of the prescription (document not required if it is a medical leave due to
maternity.

6.2.1.2 Hospitalization:
i. Hospitalization certificate (is managed upon patient admission).
ii. The medical leave in Essalud format, Certificate of Incapacity for the
Temporary Work (CITT), issued at the time of registration.
6.2.2 Medical leaves issued by private doctors

6.2.2.1 Outpatient Care:


i. Proof of payment for the medical consultation, from the same date as the start of the
medical leave (except for the staff doctor of Tecsur).
ii. Medical prescription from the date of the visit and proof of payment for the medicine
indicated in said consultation.
iii. If applicable, order of auxiliary tests (laboratory, X-rays or other)
similar), report of the results and the payment receipt.
If the medical leave is longer than 15 days, a medical report must be attached.

6.2.3 Hospitalization:

i. Settlement of expenses, issued by the clinic at the time of discharge, with the
payment receipts corresponding.
ii. Report of results of auxiliary diagnostic tests (laboratory,
X-rays or others depending on the case.
iii. Surgical report in case of surgeries.
iv. Epicrisis issued by the treating physician.

6.2.4 Maternity:

i. The medical leave will meet the requirements indicated in the point
6.2.2, including the EDD (estimated due date) and LMP (last menstrual period).
ii. Photocopy of the latest echographic report (greater than 30 weeks of gestation).
iii. Photocopy of the prenatal control card.

6.2.5 Special Cases:

• For oncological diagnoses: present the original and a photocopy of the pathology report.
• For physical rehabilitation cases: present a photocopy of the control card of
therapies, signed and sealed by the specialist.
• For chemotherapy treatments or similar: present original and photocopy of the
recipes and laboratory analysis for each chemotherapy session or similar

6.2.6 Private medical leave longer than 21 days:

In these cases, in addition to what is requested in point 6.2.2, you must attach:
• The medical report updated to the date of the issuance of each medical leave, not
having to exceed each of them by 30 days.
• Clear photocopy of the patient's ID with an address in the city of Lima. If it appears with
address in the province, they must make the change with Reniec and present us with a
photocopy to manage the change in Essalud and to exchange your medical leave
in this city.

7. Exceptions

If any exception to the following procedure arises, it must be authorized by


the Management.

8. False information

If the falsity in any presented document is proven, sanctions will be imposed.
