GUIDELINES IN AVAILING THE GPP

1. All students who agree to the Terms and
Conditions set
by Vicissitude and Student Publications Office
(SPO) and
who wish to subscribe in the GPP and Yearbook
2016 must
undergo the Vicissitude Online Registration.
2. Students who will avail the Graduation Photo
Package
will automatically be included in Vicissitude 2016
(DLSU-D
yearbook) which means that the student will also
pay for
the said yearbook.

3. Students who are considered part of the
graduating batch but failed to meet the
requirements for the graduation (including the
Summer and Octoberian graduates) may still avail
the Graduation Photo Package and Yearbook.
4. The Graduation Photo Package (GPP) and
Yearbook payment must be settled only at the
Accounting Office where an Official Receipt (OR)
must be issued. Graduating students, however,
have to secure payment slips which are already
included in the Subscription Agreement to be
printed by the student upon his/her completion of
the Online Registration. Deadline of payment
shall be on the Finals week of the First Semester.

5. Students with financial problems may submit a

promissory
form
(available
at
fb.com/vicissitude2016) to be submitted at
GMH115, Student Publications Office.

6. The OR, together with the pictorial stub, must

be presented on the pictorial schedule.

7-8. The pictorial schedule set by Vicissitude 2016
EB shall be strictly followed. However, if a student
fails to report on the indicated schedule, a letter
of request for pictorial rescheduling together with
supporting document/s shall be presented to
SPO / Vicissitude 2016 EB, otherwise, the student
shall be subjected to a penalty of three hundred
pesos (P300.00) per reschedule.

GUIDELINES IN AVAILING THE GPP

9. Graduating students are advised to bring garment for the
corporate (upper) and casual (upper and lower garments
including footwear) shots. However, they may choose to borrow
from the limited apparel (upper garment only) provided by the
photo studio.
10. For the ladies, please provide additional make-up sponges for
hygienic purposes during the make-up sessions for the pictorial.
11. For those who will bring their OWN MAKE-UP ARTISTS, please
approach the photo studio staff for the process to undergo.

12. After the pictorial, graduating students shall
be the ones to choose the shots to be printed.
Chosen shots will be written on stubs provided by
the Photo Studio and these should be kept by
students for future verification.

13. Request for group shots shall NOT be

entertained.

14. SPO shall be the authorized unit to receive

and store the GPP. The Vicissitude 2016 EB /
Photo Studio Staff / SPO Staff shall be designated
to distribute Graduation Photo Packages.

16. Storage of GRADUATION PHOTO PACKAGE and

the GRADUATION RITES PHOTOS shall be kept and
managed for only a period of one (1) year.

17. Unclaimed photos after ONE (1) year shall be

DISTRIBUTION OF GPP
1. Graduation Photo Packages shall be distributed per
college on dates that SPO/Vicissitude 2016 EB shall
announce through posters, tarpaulins, University website,
and online networking sites.
2. Graduation Rites Photos, on the other hand, shall be
released six weeks after the Graduation ceremonies.

3. The Vicissitude 2016 EB / Photo Studio Staff / SPO Staff

shall be the designated persons to release Graduation Photo
Packages / Graduation Rites Photos at GMH115 or at other
venues to be announced by the EB.

DISTRIBUTION OF GPP
3. In claiming the Graduation Photo Package / Graduation
Rites Photos, the students ID must be presented to the
Vicissitude 2016 EB / Photo Studio / SPO staff. The Official
Receipt however may be requested, by the SPO staff or EB, to
be presented, only to validate payment.

4. Graduating students with complaints / concerns (poor

quality, missing prints, empty CDs, etc.) must promptly
inform the Vicissitude 2016 EB / SPO Staff for
appropriate action. Complaints pertaining to Photo
Packages shall only be entertained up to the third
month from the distribution of graduation rites pictures.

5. A representative may claim the Graduation Photo
Package / Graduation Rites Photos on behalf of a
graduate, provided that an (1) authorization form
(available at fb.com/vicissitude2016) signed by the
student, (2) the students school ID (or any valid ID in
case

YEARBOOK GUIDELINES
1.

The

yearbook

shall

include

those

who

officially

subscribed and had completed the requirements needed

for the subscription (including those who had their
pictorials).

2. In case of siblings, parent and children, a LETTER OF

REQUEST FOR SINGLE SUBSCRIPTION shall be submitted
to SPO until January 16, 2016.

3. Yearbook requirements, specifically the Student Data Form (SDF), must
be submitted upon the completion of the Online Registration. This is only a
ONE-TIME submission which means that the student will not be able to reedit or alter anything that has already been submitted. Failure to comply,
may lead to ones incomplete profile in the yearbook. No verification of
data will be done as the file will be the basis for all information which will
be used as reference in the event of errors discovered once the copies of
VICISSITUDE 2016 are printed.
4. FAILURE TO COMPLY may lead to ones INCOMPLETE PROFILE IN
THE YEARBOOK

5. Yearbook payment must be settled before the final exam of the

first semester.
6. The Vicissitude Staff / SPO reserves the right TO PULL OUT a
graduating students picture and profile from the Yearbook, IF PAYMENT

7. The Vicissitude/SPO reserves the RIGHT TO CHANGE THE
SUBMITTED STATEMENTS (STUDENT DATA) AS THEY SEE
FIT.

8. The target RELEASE OF THE VICISSITUDE 2016 is ONE

(1) YEAR FROM THE DATE OF GRADUATION OF BATCH
2016.

9. A representative may claim the Yearbook as long as an

AUTHORIZATION FORM (available at fb.com/vicissitude2016)
and a PHOTOCOPY OF VALID IDs (WITH SIGNATURE) OF
THE OWNER AND THE REPRESENTATIVE are provided.

10. The Yearbook primarily SERVES AS MEMENTO AND IS

NOT AN OFFICIAL PROOF OF GRADUATION.
11. DLSU-D shall keep the remaining yearbook 2016 stocks ONLY
FOR A MAXIMUM OF TWO (2) YEARS FROM THE DAY OF
ITS LAUNCHING.

with your my.DLSUD username and password.
2) Read the GPP and Yearbook Guidelines; once done,
tick the Agreement Box.
3) Carefully select your preferred Pictorial Date and
Time. Once done, click the RESERVE button.
(This is a one-time reservation only which wherein a RESCHEDULE for
the pictorial date and time will not be accommodated.)

4) Fill out all the information needed for the yearbook

through the Student Data Form and click the Submit
button.
5) Print the Contract and submit it to Vicissitude at the
Student Publications Office (GMH 115).
