Case Presentation

Background Information
African American female student who is a
senior at Scotland High School and she is
17 years old, an only child and a cheer
captain on the varsity cheerleading team.
This high school senior has been diagnosed
with Post-Concussion Syndrome and General
Anxiety Disorder. Her parents have been
divorced for 3 years and her mother has
recently remarried.

Problem Statement
Her post-concussion syndrome was triggered
by a head injury during cheerleading practice
and has caused a decline in grades last year.
Her anxiety disorder causes her to constantly
over think situations and she starts worrying
about the outcomes (ex. Applying for
colleges).

Services
This client does have a 504 accommodation
plan which allows her certain academic
accommodations (ex. Extra testing time).

Strengths
Leadership with being the captain of the
cheerleading team.
Responsibility with applying for different
colleges and managing the cheers.
Academically gifted, this student has a 4.5
GPA at Scotland High School.
Participated in the Oban Exchange
program
and traveled to Oban,
Scotland for 2 weeks.

Limitations
Client is a procrastinator which
tends to make her mother
frustrated and these limitations,
at times, causes the client to
become withdrawn and begins to
have feelings of selfworthlessness.

Assessment Goals
Assessment goal is to make the client
as comfortable as possible in her class
setting without compromising her
learning ability just because she has
504 accommodation plan for classes.
In which her assessment goal plan is to
allot her extra time for curriculum and
testing and any other accommodations
she may need.

Interventions
w/Timeframes
Extended time on assignments/test
requiring more than 30 minutes.
Breaks every 30 minutes (student may
step outside of classroom to get her
thoughts together).
On assignments/tests words/numbers
should be at a 14 pt. font.
Student may use a magnifying glass or
glasses when needed for reading
test/assignments.

Questions
1. There were no biases.
2. There were no value differences.
3. Leadership skills as a team captain has
shaped her. Her task include scheduling
different team members weekly to make
the banner for the football players to run
through, helps the team work through
dilemmas, and calls cheers at the games
on Friday nights.

4. Diagnosis are a must in order for
staff to conduct a 504 meeting and
to be able to give the student the
help they need to adapt in class.
5. The only research is if we do not
understand the diagnosis we will
research to find out what it means,
what causes it, and a cure if any can
be found.
6. No changes are needed.

Client Goals
The clients goals are to graduate in the top 10 of
her class out of 358 students graduating June of
2017.
Attend college on a full ride scholarship
She has already received 2 acceptance letters for
the fall of 2017.
To keep learning how to accepting defeat. Client is
a member of 3 club and was not a nominee for
homecoming court. Dealing with dilemma helped
her to see that she had other obligations to tend to
such as the pep rally and being the captain of the
cheerleading team she had to get everything
organized.
