Patient M, male, 5 years and 3 months old, came to AA hospital at


may 8th 2011.
Chief complaint
A bump on the left scrotum since 1 week before entering to the hospital.

Present illness
history
Since 3 days after the patient
was born, his mother complained
a bump in the left scrotum and
extending to above the pubic
which often is seen when the
patient cried. Bump as big as
lizard egg. Bump is not visible if
the patient didnt cry.

Bump increasingly being seen after

patient played or cried. Not seen


when patient wake up. If bump seen,
his mother just sort the bump. Bump
can still be repositioned.

Since 1 week before entering to

the hospital, his mother complained


of bump on scrotum patients cannt
be repositioned again. Patients are
not nervous and felt no pain when a
lump in press. No complained of
fever, nausea and vomiting.
defecation no complained.

Patient was born enough

Birth weight was 2400 grams.

Palpation : defans musculer and,
tenderness not present
on all
abdominal field.

The left inguinal-genital


Inspection : visible a oval bump from

the left inguinal to the left scrotum,

size as big as lizard eggs.
Auscultation : bowel Noisy (+)
Palpation : soft bump, no tenderness,
bump
cannt be repositioned. Testis
is
palpable 2 pieces.
Special Inspection

Transluminasi (-)

DIAGNOSIS
Left hernia inguinalis lateralis irreponible

Examination of routine blood:


Hb : 10,5 g/dl
WBC : 11.700 /mm3
Platelet : 377.000/ mm3
Ht : 32,6 vol%
