CONTRACTION STRESS TEST (CST)

VIBROACOUSTIC STIMULATION
a specially designed acoustic
simulator is applied to the mother's
abdomen to produce a sharp sound
(approximatley 80 dB, 80 Hz),
startling and waking the fetus.

during a standard nonstress test, if a


spontaneous acceleration has not
occured within 5 minutes, apply a
single1 to 2 seconds sound
stimulation to the lower abdomen
Can also be used to detect fetal metabolic acidemia.

Normal: fetal heart rate(FHR) acceleration is present after


stimulation (with fetal movement)

Abnormal: absence of FHR acceleration. Only about 50% have


acidotic pH.

The presence of FHR accelerations reliably predicts the absence


of fetal metabolic acidemia
CONTRACTION STRESS TEST (CST)
If the result is non reactive, the test will be reapeated. If remains non reactive, the
mother will be scheduled for CST.
CONTRACTION STRESS TEST (CST)

The fetal heart rate is analysed in conjuction with


contractions.

To identify the fetus that is at risk for intrauterine asphyxia

Usually shows if there is placental insufficiency

Source of ocytoxin for CST is by nipple stimulation


Procedure for CST
Gentle stimulation of the nipples release oxytocin.
Electronic fetal monitor is attached
Goal : have 3 contraction in 10 minutes

Results:
negative: NO fetal heart rate decelerations were presents with
each contraction

positive: 50% or more of the contractions cause a late


deceleration
OXYTOCIN CHALLENGE TEST (OCT)
Oxytocin stimulation started intravenously
Electronic fetal monitor is attached
Goal : have 3 contraction in 10 minutes

Results:
Negative -- 3 contractions in 10 minutes with NO signs of
deceleration

Positive -- repititive persistent late decelerations for half or


more of the contractions
Example of a positive contraction stress test. Repetitive late decelerations occur
with each contraction.
