Background:
Catamenial pneumothorax (CP) is a condition of air leaking into
the pleural space (pneumothorax) occuring in conjunction with
menstrual periods (catamenial refers to menstruation), believed
to be caused primarily by endometriosis of the pleura. Usually
onset of lung collapse is less than 72 hours after menstruation.
Most commonly occurs in women aged 30-40 years.

Case:
Female, 47 years old, came with spontaneous pneumothorax. She
is in her third day menstrual. We performed right chest tube for
emergency treatment and open thoracotomy for biopsy (diagnostic)
on the next day. Intraoperatif we found bluberry spot on
diaphragma. Histopatological finding was benign lesion wih sign of
old bleeding, can support an endometriosis. She was having
hormonal therapy leuroprelin acetate 6 times for each month now.

Summary:
This was very rare case condition found in primary spontaneous
pneumothorax in women. Sometimes catamenial pneumothorax is
probably under recognized and also have high recurrence rate.
