›Type of intravenous fluid therapy

1. Isotonic
the osmolarity almost same with the plasma osmolarity.
This fluid increasing the extracelullar volume. One liter of
isotonic fluid increasing one liter of extracelullar volume.
3 liters of isotonic fluid replace 1 liter of blood loss.
Example : NaCl 0,9%; RL; Dextrose 5%
2. Hypotonic
the osmolarity lower than plasma osmolarity. The function
of hypotonic to replace the celullar fluid.
example: NaCl 45%; Dextrose 2,5%
3. Hypertonic
the osmolarity higher than plasma osmolarity
example: Dextrose 5% in saline 0,9%, D5% in RL.
BAXTER FORMULA:

Adult : 4 cc x BW (kg) x burn area (%)

Child : 2 cc x BW (kg) x burn area (%)

Example : man 50 y.o, BW 50 kg, burn area 20%

Body fluid needed: 4 x 50 x 20% = 4000 ml

First 8 hour: ½ total body fluid needed

Next 16 hour: ½ total body fluid needed
drop/ minutes = volume x drop factor
hour x 60 minutes
Drop factor:
Macro (20 drop/minutes for adult)
Micro (60 drop/minutes for child)

Example
First 8 hour = 2000 x 20 = 83 drop/minutes
8 hour x 60
Next 16 hour = 2000 x 20 = 39 drop/minutes
16 hour x 60
