Chief Complaint : lump on the left inguinal

It has been suffered by the patient since 3 years before


admitted to Pirngadi General Hospital. Lump on the left
inguinal appear at the time of coughing and straining, and
dissapears while lying down. Pain was felt intermitten on
the left inguinal. Nausea (+), Vomitting (+), Fever (-),
Decrease appetite (-), Defecation (+), Flatus (+)
Micturition (+).
Present State

Awareness : Alert
BP : 130/70 mmHg
Pulse : 76 beats / minute
RR : 20 times / minute
T : 36,8ºC
Generalized State

Head : Abnormality wasn't found


Neck : Abnormality wasn't found
Chest : Abnormality wasn’t found
Abdomen : In localized state
Genitalia : Male, abnormality wasn’t
found
Extremities : Abnormality wasn’t found
Localized State:

Abdomen :
I : Asimetric, lump on the left inguinal
P : Muscular rigidity (-), pain(-), felt mass on the
lower left abdomen quadran, finger test (+)
P : Timphany
A : Peristaltic (+) normal

Working Diagnosis :
(L) Hernia Inguinalis lateral
Management:

- Fasting
- IVFD RL 20 gtt /i
- Insertion of Urinary catheter, came out initial urine
300 cc
- Inj Ceftriaxone 1 gr
- Prepare for Hernioraphy
