Failure Date

- Defined as the first day the product was not available for use by the customer,
or the date the issue was identified and documented.

- The work order open date may be considered as the failure date.
- If failure date does not apply, i.e. before failure service letter, the date in the
repair date may be used as the failure date.

Repair Date
• Represents last day of labor on a work order
• Since Repair Date is a required field, users can enter failure date in this field
as well. Users are encouraged to edit the entry later with the correct repair
date. Note, if a claim is submitted later with the same Serial Number, Part
Causing Failure, and Group Number, the claim will update the SIMS record
with the accurate repair date.

Component Serial Number


Provides additional information related to the part causing
failure (PCF). If the PCF is serialized, enter the PCF serial
number here. (Note, this field is optional for service
reporting.)

Component Usage Information


Enter usage information specific to the PCF or related component.

Service Meter field should reflect the amount of use on the part/component which may be different from the product SMU.

Usage description field should reflect the usage value such as hours, miles, kilometers, cycles, metric tons, kilowatts, tons, or tracks,
etc.
