Greeting the patient:

Hi/hello/Good morning/Assalamu’alaikum.
My name is ............. You can call me .........
I am a nursing student of Stikes AlAIC.
I will be looking after you today.

Admitting patient:
What is your name? My name is .........
When were you born? I was born in April 1st, 1999.
Where do you live? I live in Cilacap.
Do you have any allergies? Yes I do/ No, I don’t.
What is your allergic to? Food, medicine, dust, etc
Now I will check your body temperature, pulse,
blood pressure and respiration.
Can we start it now?
Patient treatment:
Excuse me, I will check your body temperature.
I will put this thermometer to your armpit.
Can you lift up your arm? Yes, please.
Your temperature is 38C so it’s rather high.
32C so it’s rather low.
I will check your pulse. Your pulse is 110 per minute, so
it’s rather quick.
I will check your blood pressure. Roll up your sleeve,
please. Your blood pressure is 130/100 so it’s rather
high.
I will check your respiration. Your respiratory is 24 per
minute so it’s normal.
Applying Infusion

 I would like to apply infusion for you.


 I need you to clench your hand by putting
your thumb inside so i can see your vein.
 I will put this tourniquet around your arm, rub
it with alcohol, and please inhale, and...done.
 Stay steady so it goes through.
Dressing Wound

 I want to change your dressing wound to


avoid infection.
 I will open it now
 Then clean your wound
 And dress your wound with new bandage.
Closing treatment:
