 ADDRESS: Tagkawayan, Quezon
 OCCUPATION: Self-Employed
 LIFESTYLE: Positive Smoker and alcoholic
 He is negative in diabetes, heart
problem, negative for history of seizure;
postive in hypertension
 He was diagnosed of CVA in the year
2007 at Mt. Carmel Hospital under Dr.
Alincarte
 Infart; Right side affected ; 1 month of
slurred speech
 In the year 2013 he was admitted at the
second time at Mt. Carmel Hospital with
diagnosis of mild stroke with GCS 15.
 Last year he was diagnosed
hyponatremia and hypokalemia.
 He was given a maintenance of
Rosuvastatin, K-lyte, Na, clopidogrel and
clonidine
 His last check up was last September
2019
 At home in Tagkawayan around 8 PM,
he was experienced of numbness and
no verbal output noted.
 The wife gave a massage to the
affected part of the patient after a few
minutes, patient was experience of
stiffening of right side extremities and
upward rolling of eyeballs (convulsion)
 They went to District Hospital of
Tagkawayan. Their… Venoclysis started,
hydralazine ampoule, omeprazole,
metroclopramide and diazepam. With
GCS of 9.
 In their family at the patient side, his
father
 Unable to breath independently
 Both eyes are slightly reactive upon light
accomodation
 Glasgow Coma Scale: Total of 3
Eye Opening – 1 (none)
Verbal Response – 1 (none)
Motor Response – 1 (none)
