Parameter for school
classification

The parameter used for this school classification is the


combined enrolment of Kinder, Grade 1 and Grade 6 for
elementary school. For Junior High School, it is the combined
enrolment of grade 7 and grade 10. For Senior High School,
it is the combined enrolment of grade 11 and grade 12.

 For grade levels with classes less than the required minimum
number, no drawing of lots is necessary.

 To be accomplished in duplicate copies, both ORIGINAL and
with attached NSO/ PSA / Birth Certificate/ Barangay Certificate
(Photocopy of duplicate copy is NOT allowed).
 Components of MAPEH for Grades 1 to 10 shall be written
with the corresponding grade. Average per component should
not be computed and written in the Form 137. Only the average
of MAPEH as a subject, and not per component.

Diploma/Certificate shall be printed using the

 For the signatories translation: Punung-guro (Principal); Ulong-guro


(Head Teacher) while Teacher In-Charge and Officer In-Charge shall be
as it is.

 Diploma/Certificate shall be INITIALED by the Chairman of the Grade


level DCC before the signature of the Schools Division Superintendent.

 Diploma/Certificate shall be placed in brown expanded envelope with


the list of names and LRN printed outside the envelope.
 Diplomas and completion certificates of the Kindergarten
and Grade Level Graduates/Completers shall be signed by
the School Heads and the Schools Division Superintendent
(SDS) per DepEd Order No. 31, s. 2019 entitled “The
Department of Education Service Marks and Visual Identity
Manual” pages 42-46.
 There will be no checking of honors in the Division Level;
hence it shall be done in the school.
 Each school is required to accomplish the School Forms Checking
Report 1 (SFCR1) and submit to their respective Public Schools District
Supervisor (PSDS) for consolidation.

 The PSDSs shall accomplish the School Forms Checking Report 2


(SFRC2) using SFRC1 as reference.

 The DCC members shall accomplish the Consolidated Division


School Forms Checking Report 3 (SFRC3) and submit to the DCC
chair for consolidation.

2018 GENERAL REMINDERS

 The signatories of the School Forms Checking Report (SFCR)


1, 2, & 3 are the Division Checking Committee, Sub-Committee
Members/DCC Representatives, and the School Checking
Committee.
 Expenses to be incurred during the preparation and checking
of forms shall be charged to the School MOOE/local funds,
subject to the usual accounting and auditing rules and
regulations.
The Final Grade in each learning area and the
General Average are reported as whole
numbers.
Remarks in the Final Grade for every learning
area shall be Passed or Failed.

General Reminders on the Preparation
and Checking of School Forms (SFs)
(Elementary and Secondary)

 School Form 6 shall be accomplished by the School Heads


consolidating the reports of the different grade levels. The
school should have only one (1) SF6 to be submitted in 3
copies.
 Forms shall be properly, completely and neatly
accomplished. Entries in all the forms shall be legibly
written.
 Observe the correct nomenclature and order of the
different subjects in Forms 137/SF10 & Form 138/SF9.

 All List of Awards and Recognitions shall first be checked by
the School Head and Public Schools District Supervisors.
 Final checking and validation will be done by the DCC before
the approval of the SDS.

and checking of School Forms
