What areas of the body?
• Shoulder

• Ankle / heel

What areas of the body?
• Shoulder

– Most popular (MSK) region imaged by Ultrasound

– Most accessible of all the large articulate joints

– Third most common in terms of MSK pain


(after Lower Back & Carpel Tunnel)
– Good patient acceptability
(no need to undress too much!)
What structures?
• Bone (surface)

• Muscles

• Tendons

• Ligaments

• Joints

• Cartilage

• Bursa

• Nerve Bundles

• Blood vessels
What pathology?
• Bone – erosion (Arthritis), crack (Rib), healing (fracture)

• Muscle injury – tears, haematomas (inter & intra), Myositis, oedema


What pathology?
• Tendon disease – tear, tendinitis, tendinosis, tenosynovitis, calcification

• Joint instability - collateral ligament tear, dynamic changes, tendon failure

• Impingement – tendon, joint, fat pad, bursitis, nerve, orthopaedic HW


What pathology?
• Mass lesions, cysts, fluids - Ganglion cysts, tumours, soft-tissue sarcomas

• Infection disease - septic arthritis, synovitis, bursitis, abscess, osteomyelitis

• Foreign bodies / orthopaedic HW


What technologies?
• High frequency

• High resolution

• (mainly) Linear probes

• Panoramic imaging

• Spatial compounding

• Color / Power Doppler

• Needle guidance

• 3D / 4D imaging

What skills?
• Good knowledge of anatomy & 3D imagination

• Good probe manipulation skills


What skills?
• Good MSK specific scanning techniques
– Recognize structures (tendon, ligament, nerve, bursa, blood vessel)

– Eliminating (or taking advantage of) artifacts


What skills?
• Good dynamic imaging skills (movement during scanning)

Posterior Labral Tear

• Good needle guidance skills


Who might be interested?
• Radiology - high end, multi application system
• Rheumatology - application specific, US = 3rd arm
• Sports medicine - application specific, portable (?)
• Orthopaedic surgery - Good at MSK US, but prefer to cut !
• Physiotherapy - “I don’t diagnose” mentality - need change
• Paediatric hip - well established, but small market
• Podiatry - still in its infancy . . .
• Plastic / maxillo-facial surgery - botox guidance ! . . . yes please !!
