HOME VISIT

Home Visit

• a family-nurse contact
• allows the health worker to access the home and family situations
• in order to provide the necessary nursing care & health related
activities
Purpose:

• G-ive nursing care to the sick, to post partum mother, and her newborn
• A -ssess the living condition to provide appropriate health teaching
• H-ealth teachings regarding prevention and control of disease
• E-stablish close relationship between the health agencies and public promotion of health
• M-ake use of the inter-referral system and to promote the utilization of community services
PRINCIPLES
• Assemble record first before home visit
• —-list of names
• —-study the case
• —-have written NCP

2. Planning-use all available information(patient and family records)
3. Give priority to the essential needs
4. Involve the individual and family in planning and delivery
5. Must be flexible
STEPS IN CONDUCTING HOME VISIT
• 1. GREET - greet and introduce yourself
• 2. PURPOSE - state the purpose of the visit
• 3. OBSERVE - observe the patient and determine the health needs
• 4. BAG TECHNIQUE - put the bag in a convenient place to perform bag technique
• 5. PERFORM AND TEACH- perform the nursing care needed and give health teachings
• 6. RECORD - record all important data, observation, and care rendered
• 7. APPOINTMENT - make appointment for a return visit
GUIDELINES TO CONSIDER REGARDING FREQUENCY OF HOME VISITS

• (Theres no definite rule on the frequency of home visits)


• Factors to consider:
• The physical, psychological, & educational needs of the individual & family
• Acceptance, interest, & willingness
• Policy of a specific agency
• Other health agencies & number of health professionals involved
• Careful evaluation of past services
• Ability of patient & family “needs, knowledge of available resources, & use of their resources
