● Manifestations initially appear 6- 12 hours after ingestion ● Potentially life-threatening Three basic types of delayed-onset mushroom poisonings:
1. Gastrointestinal/ headache/ liver
damage ● Noted toxin: Monomethylhydrazine (MMH) ● Bloated feeling, nausea, vomiting, diarrhea, abdominal pain, headache, muscle cramps, and vertigo. 2. Gastrointestinal/liver damage ● Noted toxin: Amatoxins ● Three Stage Progression: 1. 12-24 hours after ingestion ■Vomiting, diarrhea, severe abdominal pain (persists for 24 hours) 2. Symptoms remit and patient seems to recover. Premature hospital release has resulted in death. 3. Symptoms resume with increased severity and are accompanied by symptoms of liver failure & failure of other organs. Death may occur in 4 to 7 days. 3. Kidney, later liver damage ● Noted toxin: Orellanine ● Delayed onset of 36 hours to 14 days. ○ Kidney failure symptoms are noted first ○ Extreme thirst, Frequent urination, pain in the lumbar region, chills or fever, diarrhea, or constipation. TREATMENT Rapid Onset Type - IV ATROPINE 1-2 mg
