What is a Safety Intervention?
• A discussion by one of more people with an individual or a group
regarding an observation of a potential unsafe act, practice or
procedure.

Why is a Safety Intervention Necessary?


• To provide safety environment to everyone.
• To prevent an accident / injury / fatality.

Who Performs a Safety Intervention?


• ANYONE who sees a potential unsafe act or condition.

Alluvion Walkway Intervention

 You are walking through the city, along a walkway between buildings and you come
across this?

Stuart Calvert, BHP Project team member working in
Perth noticed something wasn’t quiet right on the
walkway leading into the Alluvion building, a couple
of week’s ago on a Sunday. And he chose Option D.

• He noticed that the concrete slab walkway over


Mercantile Lane (between Westralia Square and
the Alluvion building) had dropped approx. 50mm.
• He found the contact details for Building
Management and got in touch with the Building
Engineer.
• He contacted security and the City of Perth
Rangers, who arrived on site within 15 minutes, and
barricaded the area.
• A Spotter/Security guard was on site directing
pedestrians away from the area.

Catastrophic failure occurred at 3:30pm, on Monday.
Just 24 hours later……….
