There are three forms of sleep apnea:

Central (CSA),
Obstructive (OSA), and
Complex or mixed sleep apnea

(i.e.. A combination of central and obstructive) constituting 0,4%, 84% and 15% of
cases)

The severity of sleep apnea is determined by dividing the number of episodes of


apneas and hypopneas lasting ten seconds or more by the number of hours of
sleep. The resulting number is called the Apnea-Hypopnea Index, or AHI.

AHI# Condition

Below 5 No OSA

5-15 OSA

15-30 Moderate OSA

Over 30 Severe OSA

• Overweight
• Deformities of the nasal septum or nose
• Pregnancy
• Polyps and cysts
• Malignant tumor
• Nasal congestion
• Swallen adenoids and tonsils
• Swelling of the muscular parts in upper airways
• Use of alcohol, antihistamines or sleeping pills

• Laser Treatment Option:


● Non ablative Er,Cr:YSGG tightening of uvula, soft palate and
surrounding tissues.
● No anesthesia – no needles

● No pre- or post-op pain


● Three 15-20 minute sessions over 2 months
● Immediate results from the 1st treatment

Treatment method
A fractionated laser beam is used at minimally invasive
settings:
- The laser beam is fired at soft intraoral tissue with
repetition rate of 5 Hz
- The laser beam is manually delivered across the target,
either vertically or horizontally (depending on the
region)
- 10 passes are performed across each region (with 50%
overlap)
- The treated tissue is thermally processed and
consequently, it shrinks

Treatments were done with Er,Cr:YSGG 2790nm laser,


using
fractional handpiece with following settings:
• Frequence = 5 Hz
Power= 2W
• Overlap: 50%
• Passes: 10 per area
(if needed, finish with additional 2)

• Sessions: 3, two weeks intervall

Mallampati classification

Class 1: Full visibility of tonsils, uvula and soft palate


Class 2: Visibility of hard and soft palate, upper portion of tonsils and uvula
Class 3: Soft and hard palate and base of the uvula are visible
Class 4: Only Hard Palate visible

• Interview (Complete the Questionnaires, describe the procedure
and post-treatment period, establish possible contra indications,
establish treatment schedule 1-15-45 days)
• (Polysomnograph – if possible)
• Photo of oropharynix (definition of Malampati type)
• Protection of the patient and the stuff (laser goggles)
• Irradiation with Er:YAG laser
• Photo of oropharynix
