INSTRUCTIONS

1. Store all files for submission in one Google Drive folder. Name it as “SUMMATIVE
ASSESSMENT WITH STUDENT WORK SAMPLES (Q1, SY 2020-21)”. This will be the main
folder to be shared with us.

2. Within this folder, create one folder per grade level and name each accordingly
(“Grade 7”, “Grade 8”, etc).

3. Within each Grade Level folder, create one folder per learning are name each
accordingly (“English”, “Science”, etc.)

4. To each Learning Area folder, upload one (1) Written Work (WW) and one (1)
Performance Task (PT), including scoring guides/ rubrics. Make sure that these are in
PDF.
5. For every Written Work and Performance Task, upload three (3) graded student work
samples that represent different student learning achievements (low-performing, average-
performing, high-performing). Name each sample as follows:

For Written Works: WW_(Learning Area & Grade Level)_SWS-(LP for "low-performing", AP for
"average-performing", HP for "high-performing")

Example: WW_Math8_SWS-AP

For Performance Tasks: PT_(Learning Area & Grade Level)_SWS-(LP for "low-performing", AP
for "average-performing", HP for "high-performing")

Example: PT_Science6_SWS-HP

6. Share your main folder by posting the link (shortened, if possible) in the space provided
below. Check the settings to ensure that the folder is easily accessible
REMINDERS

1. For MAPEH, there should be individual folders for Music, Arts, PE and Health.
This applies also for each component of EPP/TLE.

2. Submissions shall be done per learning area per grade level, not per teacher.
For example, if three teachers are teaching English 4, they can decide together
which WW and PT to submit, and upload just one of each.

3. To maintain anonymity, all student work samples must not contain any identifier
(e.g. name, section).

4. Videos as student work samples are accepted, but make sure that informed
consent is obtained from learners involved.
