CHF CLINIC WAITING ROOM
Have you been diagnosed with Heart
Failure?
Do you continue to experience symptoms of shortness of breath, edema, and fatigue?

Have you been hospitalized as a result of your heart failure in the last 90 days?

Do you feel that you have a decreased quality of life due to your diagnosis?

Are you over the age of 45?

Important information before inquiring about the study
• NO COST to participants
• Participants will be required to keep a medication log and perform daily
weights and document
• All scales and medication logs will be provided
• Accurate logs must be kept in order to obtain valid data
• Patients will be required to complete two questionnaires monthly during
study
• An Echocardiogram will be done every 6 months until end of study to
evaluate heart function
• If you have Chronic Kidney Disease requiring dialysis or suffer from
uncontrolled diabetes, you do not qualify for this study
