DNA FRAGMENTATION
• Spermatozoa are highly specialized cells
to deliver an intact paternal genome to
the oocyte.
• DNA fragmentation is the presence of
DNA breaks.
• Some research discovered that high
levels of sperm DNA fragmentation
(sDF) can be found in spermatozoa of
sub/infertile men..

DNA FRAGMENTATION
• About 50% of all cases  underlying cause for a pathological semen analysis cannot be found.

• In addition, in about 15% of all couples with subfertility/infertility caused by male factors, standard semen analysis
does not reveal abnormalities.

• One of the reasons may be related to sperm DNA integrity.

• Recent studies have shown that DNA integrity plays an important role during fertilization and early pregnancy.

• DNA damage can be caused by intrinsic (e.g. pathological maturation of sperm cells, oxidative stress) or extrinsic
factors (e.g. varicoceles, infections or advanced age).

HOW TO MEASURE DNA INTEGRITY?
1. Sperm-Hyaluronan Binding Assay (SHBA).
• Indirectly tests DNA integrity
• a method based on the premise that only fully developed mature sperm cells with high
DNA integrity have the ability to bind to hyaluronic acid, which simulates the human
pellucid zone.

2. Sperm Chromatin Dispersion (SCD)


• Proven to be an effective tool to directly assess DNA integrity.
• This test detects the ability of the sperm chromatin to disperse, following acid
denaturation and removal of nuclear proteins.

DNA Fragmentation Index
(DFI)
• The degree of DNA integrity is determined by the DNA fragmentation
index (DFI).

• The DFI is the percentage of sperm cells with DNA damage.

• A DFI value above 15% is commonly understood to be pathologic and


is associated with subfertility/infertility.
– However, so far 15% is only a threshold that still warrants more research.

DNA Fragmentation: Correlation with Embryo Development, Lower
Implantation Rate and Miscarriage Rate

• In 2019, Borges et al reported that higher SDF is correlated with poor embryo
development, lower implantation rate, and higher miscarriage rate in non-male
factor infertility ICSI cycles.
• Based on the prospective cohort study of 475 cycles of IVF, he concluded that
SDF test can bring additional information to sperm quality evaluation of men

DNA Fragmentation Testing (2013)
Summary of Recomendation
There is a fair evidence (Level B) that increased DNA Fragmentation is associated with
reduced fertility; however, there is insuficient evidence (Level C) to use the test as a
predictor of fertility since cut-points have not been clearly established and validated.
There is insufficient evidence (Level C) to recommend the use of DNA integrity tests
to predict pregnancy with IUI
There is insuficient evidence (Level C) to recommend routine use of DNA integrity
testing for patients undergoing IVF

sDF as A Predictive Parameters in the Outcomes of
ART Cycles in Women with Reduced Ovarian
Reserve (ROR)
• From a total 2,865 of first cycle of IVF or ICSI, the researcher reported that SDF above 27.3% might
negatively impact the live-birth and implantation rates in the ROR Group but not in the NOR (normal
Ovarian reserved) group.
• However, this retrospective data also reported that SDF above 27,3% significantly increased the risk of
early abortion in the NOR group

Varicocele Repair
• Higher levels of SDF were observed in patients with
varicocele.
• Elevated levels of ROS damage both nuclear and
mitochondrial DNA causing base modifications, strand
breaks, and chromatin cross=links as well as increasing
sperm DNA Damage
