•BY KNOWING THE REASON THE MANAGEMENT CAN ABLE TO
TAKE CERTAIN MEASURES TO PROTECT THE EMPLOYEE
DIFFERENCE BETWEEN THE SURFACE AND THE
ROOT CAUSE

• The cause of the incident can be described by plant, place, competence, and system
• Root cause also describes the main reason for the accident (Lopez-Arce et al., 2020)
• The root cause helps the investigator to know the proper reason
• The surface cause is the hazardous condition of the workplace that contribute to the
accident
DESCRIBE THE 5 “WHY” PROCESS

• The first activity describes why the person is falling


• The second activity describes the time-line of the event (Almuntaser, Sanni-Anibire &
Hassanain, 2018)
• The third activity define the intervention point of the timeline
• The fourth activity explain that why a person not wearing the protection kit
• The fifth activity describe what measure can be taken to stop these incidents
EXPLANATION OF THE CONCEPT OF THE LESSON
LEARNED PROCESS

• The idea lesson learned is a concept which tells what knowledge has
learned in the project of investigation of falling off a person
• The evaluation of the mistakes also have to do in this process (Ajmal,
Malik & Saber, 2017)
• The positive aspects also analyze in this process
• The main idea is to not repeat the mistakes again in the project
