 Chief Complaints
• Itching and blisters in palms and foot, painful
erruptions
 History of Present Illness
• 1 month PTC patient started itching in palms
and soles, and all ?
• Itching is so intense, there is scratching, and
bleeding painful blisters since a week ago
(when?)
• He tried using guava leaf for the itch to get
relief
• Today (when?) he has multiple erruptions and
there are blisters in his palms and soles of ?
Size
• There are clear fluid discharge from the
blisters
• The itching aggravates at night and relieved by
putting hot water and guava leaves
 Past Medical History
• Patient was given Amoxicillin 500mg for the ?
Condition but wasn’t relieved by that
