 SCENARIO
An 12-year old female patient has just been
initially diagnosed with pneumonia by the attending
physician. Initial data upon the assessment shows
that the patient has tachypnea, on & off fever for 1
week; cough and colds for 2 weeks, use of
accessory muscles when breathing and wheezing
sound upon auscultation. You also noticed that the
patient is weak and pale in appearance.
 The physician ordered an IV Fluid of PNSS with a flow rate
of 62-63 ugtts/min to run at 16 hrs. The AP also ordered the
following medications: Cefuroxime 300 mg in 10 mL of sterile
water IV q8; Acetylcysteine 1 effervescent tab BID;
Paracetamol 100 mg IV q6 for fever; Salbutamol ½ neb with
2cc of sterile water q12; She has an order of CBC; CXR AP,
Na, K, Cl; UA & FA; O2 Therapy of 1-2 LPM as needed; HBR
at bed. When you informed the patient’s mother to buy the
medications ordered, she said, “Baka pwedeng dae ko na
bakalon ang iba, ta ito nganing aki kan kapitbahay mi narahay
man na dae nagiinom nin bulong”.

 SCENARIO
Day 2 of your clinical exposure at 7-1 pm shift. You
were assigned to the same client as yesterday.
During the endorsement and initial rounds, you’ve
noted that abdominal pain is still evident and he
manifested only 1-2 semi-formed stool. When
asked how he was, he just touched his abdomen.
The mother added, “Makulog pa daa an tulak niya
pero bako nang arug kaitong kasuhapon”.
