• DepEd Commons Account/ MS Office Account
• Adobe Reader DC in your computer • Passport size picture saved in your computer • Signature saved in your computer 1. Open lms.deped.gov.ph Then click Toledo City (as shown in the picture below) 2. After clicking Toledo City, click Deped Commons Teachers 3. You may continue with your Microsoft Account or Commons Account 4. Select Region 7-2 and sign in to your account 5. Once directed to Learning Management System (LMS) portal, click Site home 6. After clicking Site home, click Professional Development 7. Then click Facility for the Submission of the Application Requirement for the PNPKI Digital Certificate 8. Enter Enrolment Key: PNPK1_Toledo Then click Enrol me 9. You may now download your PNPKI Form 10. Once downloaded, click the arrow beside the file and click Show in folder 11. You can see your PNPKI Application Form and make sure your computer has ADOBE READER DC to edit your file. 12. This is what the form looks like.
