Section 4: Identification Card

1. Issued university Identification Card

2. A student must secure the prescribed university ID.
3. The university ID shall be validated every semester.
4. Prescribed lanyard
5. Tampering and using other ID is strictly prohibited with
extreme prejudice.
6. Alteration to the university ID is prohibited.
7. Manufacturing and usage of falsified ID.
8. Lost ID must be reported immediately to Student Affairs
Office
9. Affidavit of loss and Form of Application.
10. A student might be ask of his/her ID for transactions.
Section 5: Prescribed Uniform
1. All bona fide students should wear their
complete uniform.
2. A student must wear university specified
uniform.
3. The university uniform must be worn properly
both inside or outside the campus.

6. Other sets of uniforms like PE and NSTP
