 The form contains information such as:

(Urban Lands)
Contents:
 For each class and sub-class of property the form
arrange the adjusted unit sales value under column 5 of
GR Form No. 4 in ascending order.
 The form also shows the determination of the unit base
market value using the mean and average method.

Buildings: Part C Extra Items as Component Parts of Building
Contents:
 The form shows extra items of building and its
corresponding construction cost which should be added
to part A of G.R. Form No. 12. It includes items such as
carport, mezzanine, porch, balcony, basement,
pavement, floor and wall finished, etc.
