Sindrom Peningkatan Tekanan Intrakranial
1. Nyeri kepala
Nyeri kepala pada peningkatan TIK dirasakan sebagai nyeri tumpul, terlokalisir dan intermiten serta mengalami pertambahan
dalam derajat beratnya. Mual muntah biasanya menyertai nyeri kepala ini pada sebagian besar pasien .Nyeri kepala disebabkan
oleh adanya traksi pada arteri cerebral besar dan sirkulus willsi, traksi pada sinus vena besar dan bagian proksimal cabang
kortikal, distensi pembuluh ekstrakrannial, dilatasi ventrikel III, inflamasi daerah yang sensitif nyeri pada struktur intra dan extra
cranial (Mardjono, 2006).
2. Mual dan muntah
Mual umumnya merupakan efek massa yang besar dan pergeseran serebral. Mual muntah yang rekuren pada dini dan malam
hari sertaa muntah proyektik tanpa didahului rasa mual memperbesar kecurigaan suatu masa intrakranial, muntah dapat terjadi
selama berlangsungnya nyeri kepala berat. Muntah yang lama yang timbul mendadak, berlansung berjam jam dan mengalami
remisi spontan, dapat menunjukan adanya perdarahan intra tumor, obstruksi saluran serebrotransien, atau peningkatan tekanan
intrakranial sementara. Secara khusus, muntah lebih sering terjadi pada massa yang letaknya di fosa posterior karena adanya
stimulasi langsung pada pusat muntah batang otak atau oleh karena hidrosefalus (Mardjono, 2006).
3. Pandangan Kabur
Pandangan kabur disebabkan karena adanya papiledem yang juga merupakan salah satu gejala dari tekanan tinggi intrakranial.
Tekanan tinggi intrakranial akan menyebabkan oklusi vena sentralis retina, sehingga terjadilah edem papil (Mardjono, 2006).
4. Penurunan Kesadaran
Gejala utama pada pasien ini adalah gangguan kesadaran. Kesadaran dipengaruhi oleh transport elektrik di otak, brain stem,
diensefalon, hemisfer serebri, serta sel neuron. Brain stem dan thalamus merupakan kunci dalam menggalakkan kesadaran. Pada
proses kesadaran, input sensorik masuk ke brain stem, mengaktifkan ascending reticular activating system (ARAS) kemudian input
akan diteruskan ke midbrain dan kemudian dihantarkan ke korteks serebri dan medulla spinalis untuk dapat memberikan
feedback. Gangguan pada brain stem (ARAS), midbrain dan thalamus menyebabkan gangguan kesadaran (Adam, Z., 2001).

• Used in acute setting for reduction of increased ICP. Proposed mechanisms in lowering ICP include following:
(1) suppression of cerebral sodium uptake, (2) carbonic anhydrase inhibition resulting in decreased CSF
production, and (3) inhibition of cellular membrane cation-chloride pump, thereby affecting transport of
water into astroglial cells. Dose must be individualized.

• 20-40 mg/d IV/IM given slowly; depending on response, administer at increments of 20-40 mg, no sooner
than 6-8 h after previous dose, until desired diuresis occurs

The anterior cerebral artery (ACA) is one of a pair of arteries

The two anterior cerebral arteries arise from theinternal
carotid artery and are part of the Circle of Willis.

Middle cerebral artery
• The middle cerebral artery (MCA) is one of the
three major paired arteries that supply blood to the
cerebrum. The MCA arises from the internal carotid
and continues into the lateral sulcus where it then
branches and projects to many parts of the lateral
cerebral cortex. It also supplies blood to the
anterior temporal lobes and the insular cortices.
• The left and right MCAs rise from trifurcations of
the internal carotid arteries and thus are connected
to the anterior cerebral arteries and the
posterior communicating arteries, which connect to
the posterior cerebral arteries.
Middle cerebral artery
Areas supplied by the middle cerebral artery include:
• The bulk of the lateral surface of the hemisphere; except for the
superior inch of the frontal and parietal lobe (
anterior cerebral artery), and the inferior part of the temporal lobe.
• Superior division supplies lateroinferior frontal lobe (location of
Broca's area i.e. language expression)
• Inferior division supplies lateral temporal lobe (location of
Wernicke's area i.e. language comprehension)

Posterior cerebral artery
• The posterior cerebral artery (PCA) is one of a pair of
blood vessels that supply oxygenated blood to the posterior
aspect of the brain (occipital lobe) in human anatomy. It arises
near the intersection of the posterior communicating artery and
the basilar artery and connects with the ipsilateral
middle cerebral artery (MCA) and internal carotid arteryvia the
posterior communicating artery (PCommA).

The cortical branches are:

• Proses cerebellum: tidak dapat melihat ke samping, pupil normal

Terapi Dislipidemia
Dosis: 25 mg tiga kali sehari. Dapat diberikan 4 hingga 8 minggu bergantung pada efektifannya.
Dosis tambahan: 50 mg – 100 mg tiga kali sehari.
Dosis maksimum: Untuk berat badan 60 kg atau kurang menggunakan dosis 50 mg tiga kali sehari.
Untuk berat lebih dari 60 kg dosisnya 100 mg tiga kali sehari.
