The Activities Claim Documentation Request Claim Survey Claim Reporting Documentation Complete filled claim form Original Marine Cargo Insurance Policy / Certificate of Insurance Original Bill of Lading, Air Way Bill, Road Contract of Carriage, and provide also the reverse page Original or copy of Invoices, Packing List, Weight Notes Documentation - continue Survey Report or other documentary evidence to show the extent of loss or damage, such as: Master’s Note of Protest Non Delivery Certificate (Except Bewijs) or Cargo Damage Survey Report (Claim Constatering Bewijs) from shipping company Statement of Facts, Tally Sheet and Stripping Report Documentation - continue Police Report (for traffic accidents or crime related claim) Container Interchange Receipt / Equipment Interchange Receipt Claim letter to the carrier/shipping agent/forwarder or other parties regarding their liabilities and all the correspondence exchange Photographs Quotation for Repair or Replacement Survey – The Purpose To establish the cause of loss or damage To assess fairly the degree of loss or damage To provide a complete picture of the situation of loss or damage Survey – On desk preparation Contacts the claimant directly or via their broker for survey schedule Ask the claimant to prepare all the available documents related to the damage cargo to be available during the visit Ask the claimant to invite the shipping agent or forwarder or any related party involved to the claim for a join survey Check the policy condition, period, premium payment etc. Prepare the blank report, camera, batteries, safety gears, plastic bags (for taking samples), markers etc. Survey - Interview Introduce your self to the claimant Explain the purposes of the claim survey Ask the claimant to present the requested documents Assess briefly the available documents Interview the claimant and focus with the questions related to the damaged cargo Survey – In the Field Check and note the type of cargo and the packaging (containers, pallets, boxes, drums, bags etc.) Check and note the condition of the outer and inner packaging for any abnormalities Check and note all the available marks, numbers & labels (shipping marks, handling marks, manufacturer’s labels, case numbers, custom’s seal etc.) Survey – In the Field Checked and write down the details of the damaged cargo (type, brand, manufacturer, year, serial number etc.) Take pictures as many as possible (the outer and inner packaging, the whole cargo, the damaged cargo, marks, numbers, seals, registration etc.) Take samples randomly and bring them to labs for testing if necessary. Ask the claimant to preserve the damaged cargo by storing to a well secured and safe area. Reporting The report for marine cargo claim survey is a standard from Lloyds Survey Agents and also called “Havarie Report”. This report is widely used almost in all over the world This report is usually pre-printed and can be prepared by filling out the survey data to the report based on the available documents and findings during visit The most important part of the report is the cause, extent and the amount of loss or damage to the cargo Please bear in mind that the surveyor shall not comment on any liability of the marine insurance policy in the report.
