INCIDENT CONTROLLER

- Get a call from witness about incident
- Rush to location of incident
- Take victim out to safe place
- Call medical team/ first aid team (co-ordination of activity)

- Inform Site Main Controller (SMC) and follow trail

SITE MAIN CONTROLLER
Responsibilities…
- Get call from incident controller about incident
- Rush to location of incident & assess severity of incident
- Make decision to operate siren
- Inform security to operate siren
- Instruct incident controller & safety officer about action to initiate
- Come to emergency control room (Security office at the main gate)
- Inform to top management, HR Head about incident
- Inform externally (GPCB, DISH, Collector, Police, Surpanch etc) from
control room
- Collect status of victim from medical team
- Collect status of event from incident controller
- Collect status from security
- Collect head count status from Head (HR)
- Declare all clear after satisfaction about controlled situation
- Inform security to operate all clear siren

HEAD - HR
Who - Brijesh Patel

Responsibilities…
- Receive call from SMC
- Come to emergency control room
- Determine Head count on record
- Co-ordinate with security
- Follow instruction from SMC and provide any assistance
- Remain available in ECR
- Collect data from assembly point controller regarding
actual head or missing any one
- Update status actual and recorded head counts to SMC

HEAD - SECURITY
Who - Rajendra Singh
Responsibilities…
• Receive a call form SMC
• Operate siren after intimation from SMC (emergency or all
clear)
• Close the gate and restrict movement
• Keep driver ready in company’s vehicle
• Depute one guard at incident site for assistance and
control the situation
• Depute one guard at assembly point for assistance
• Control incident site as per instruction from incident
controller
• Follow instruction from HR Head
• Back to normal work after all clear siren

SAFETY TEAM
Who - Siddharth Shukla
Responsibilities…
• Receive a call from IC
• Rush to site with team
• Assess situation and assist IC for co-ordination of activities
• Depute one safety officer/ fireman at location of incident
• Come to emergency control room upon SMC call
• Co-ordinate activities and provide assistance to SMC
• Provide safety gadgets to team for rescue operation, fire
fighting and other activities

ASSEMBLY POINT CONTROLLER
Controls mass at Assembly Point (one at a time)
Who - Nirmal Parmar
Responsibilities…
- Act after siren listening
- Rush to assembly point
- Co-ordinate mass to remain in Q
- Perform head count (Q-count)
- Determine missing head
- Provide data of actual head to HR Head in ECR
- Follow instruction from Site Main Controller
- Allow mass to disperse back to their work after all clear
siren

HEAD – MEDICAL SERVICE
Provides medical assistance to victim
Who - Dilip Sindhal
Responsibility
• Receive call from incident controller
• Rush to site with necessary equipment
• Provide medical treatment to victim
• Take victim to OHC (if required)
• Co-ordinate with external hospital
• Take victim to hospital (if required)
• Remain up-to-date with victim status
• Provide status to SMC
• Follow instruction from SMC
• Update status of victim to SMC
