FAQs on Tally Sheet
• How to allocate House Hold number? What is one House Hold?
• The definition of House Hold (HH) is where people live together and eat together. Therefore
House Hold should be based on kitchen and each HH should be allocated separate HH number
as per tally sheet. If there is compound and more than one HHs then each HH should be
allocated separate HH number and combined door marking can be done on main entrance but
with separate HH number.
• What is the total number of children for that HH?
• Total number of children 0-11M and 12-59M of age are those living in that HH. Servant’s
children should also be included if they are attached with similar kitchen. Please note that
guest children should not be included in total number of children.
• Which children should be recorded in Vaccinated 0-11 and 12-59 months column?
• Team should record all those children;
• Received two drops of polio vaccine by same team
• Already Vaccinated by any other team and Having finger mark for the same campaign
• New Born (if vaccinated)

• How to record vaccination status of Guest Children in G column?
• Number of guest children vaccinated by this team should be recorded in G column
at the bottom of Tally Sheet. Guest children are those who do not belong to this HH
and these children should not be included in total number of children. Only
Vaccinated Guest should be marked in the column of Guest children.
• How to record Guest NA or Refusal?
• Guest NA or R can be recorded on back of tally sheet (space available for recording).
This recording is only for FLW to follow up. Guest NA and R will not compiled in form
2A or 2B
FAQs on Tally Sheet
• How to record lock house?
• Team should record lock HH as L on front side of tally sheet and if FLW don’t know the information then
either ask information about the HH from neighbors or consider previous door marking. Team should
record all locked HH on the back side of tally sheet with information provided by neighbors.
• If neighbors told that there are ‘X’ number of children living in this house, team should record those ‘X’
number as NA on the back of tally sheet.
• If neighbors told that there is no child living in this house even then this HH should be recorded on back of
tally sheet (for revisit and confirmation) but the number of children should be L/0 and not counted as NA.
• If the neighbor/community told that this HH is permanently shifted from this location then this HH should
be considered as permanent lock HH with no children i.e. 0/0.
• If FLWs found children in L/0 HH on same day catchup it can be corrected on Tally Sheet. If FLW found
children in L/0 HH on catchup day it can be vaccinated and mention as unrecorded missed children (street
children)
Note: we can not consider Lock HH as 1 NA. FLW should get information by any mean or take support from
AIC or Local Influencers.
FAQs on Tally Sheet
• How to record children vaccinated in School and Madrassa?
• Children vaccinated at school or madrassa can be recorded in front of S/M columns. It
can be recorded as 12/15 i.e. out of 15 total 12 are vaccinated
• How to record NA or R children at School or Maddrassa?
• There is no need to record NA or R at School or Maddrassa because such children will be
either at home or can be recorded as NA/R at home. A Refusal School or Maddrassa
should be communicated immediately to supervisors.

• Can we include all children vaccinated in streets as ‘M’ category?

• How to record unrecorded missed children on catchup day?

• How to identify persistently missed children?

• What is G1, G2 and G3 for micro planning?
