Health

The absence of disease or illness.

Safety

Control the losses of organization to isolate the danger from worker and vice versa.
Safety is a common sense: Look, Think, Done to save our self, others and property
Stay Alert From Each Task You do.

Environment

The surroundings of our living and working place which includes air, water, animals and others.

Hazard

Anything that has potential to cause harm to PEME(people, environment, material, equipment).
Risk

The chances that a hazard will cause harm together with its severity.
Or
Working without safety measure cause risk

Accident

An unforeseen, unwanted and unplanned event that cause harm to PEME.

Incident

An unforeseen, unwanted, unplanned event that does not cause harm to PEME.
Or
Anything which could have cause of accident

Near miss

Any form of accident which could result in injury or loss but do not.
Confined space
The space which has limited entry and exit is called confined space.
Or
The space which is not design for continuous occupancy
e.g. tank, vessel, pit, excavation deeper than 1.2m.

Fire

Fire is a chain reaction of three things


Fuel
Specific heat
Oxygen
oxygen

Fire control method
 Oxygen control method is called Smothering.
 Specific heat control method is called Cooling.
 Material(fuel) control method is called starvation.

Control measure

 Fire should not happen.


 If it is happen , it should not spread out.
 If it is spread out then losses should be minimum.
Classes of fire

Class A: ordinary combustible solid(paper, plastic)


Class B: liquid(grease, diesel, petrol)
Class C: Electric fire
Class D: metallic(iron, magnesium)
Class K: kitchen(cooking oil)

Types of fire extinguisher


Water for class A
Foam for class A & B
Carbon dioxide for class B & C
Dry chemical powder for class A, B and C.

PASS
For extinguisher we should use pass methodology
P: pull out the safety pin
A: aim at fire
S: squeeze the handle
S: sweep side by side
Risk Assessment
Before start of the job we do some assessment that is called risk assessment.
It is multiple of likelihood/chances and consequences.

 Feel
 Smell
PTW
Permit to work is written agreement between contractor and client.

Kinds of work permit


Confined space entry (2) hot work permit (3) cold work permit

LEL

Lowest percentage of a gas present in air is capable to produce flash of fire in the presence of ignition
source
Max range is 10%.
Oxygen deficiency and enrichment
19.5 to 23.5
Less than 19.5 is suffocation and greater than 23.5 in oxygen enrichment
Here range is 20.8 or 20.9
Excavation/Trench

Digging of soil by crane or manually is called excavation.

Hazards
 Flooding in case of digging
 Falling hazard
 Electric shock
 Underground cables and pipe
 Slip trip
 Cave in(soil can collapse from side
Controlling Measure
 Permit to work
 Inspection of soil condition
 Inspection of excavation area by competent person
 Barricade that area
 Wearing proper PPE
 Display sign board
 Hand tools should be placed away from the excavated area
 Emergency plan
 Parking of vehicle at 10 feet distance
 Minimum two exit and entry point
 If excavation depth is increased by 1.2m that is called confined space

Scaffolding

That is not permanent platform


Scaffolding Tags:

We use three types of tags for scaffolding


(1) Green: It means that scaffolding is safe to climb
(2) Yellow: Scaffolding is not safe, full body harness is required.
(3) Red: Only scaffolder’s can climb

Confined space hazards


 Oxygen deficiency
 Gasses may available
 Temperature
 Falling objects
 visibility
Control measure
 Get confined space permit
 Check oxygen level
 Check gasses and gas test
 If oxygen level is less than 19.5% then we will use mechanical ventilation
 The person who will perform work in confined space he must fully trained
 Whole watch is required for confined space
 He must be record all the names of people who enter or exit from confined space
 Supervisor is responsible for the work being performed in confined space
 provide light if needed
