What is E-CoopRIS?
• a web-based information system that provides CDA registration
officers an online platform for cooperative name reservation,
evaluation and approval of application for new registration,
amendments and authorization of branch/satellite cooperative offices
as well as recognition of laboratory cooperative.

• stands for the Electronic Cooperative Registration Information


System. It is an enhanced, effective, efficient and computerized
registration processes in granting juridical personality to qualified
prospective cooperatives and other registration related services to
cooperatives.

Step 1. Account Creation
One email-one coop policy.

● If members personal email address will


be used, ensure that access to the e-
coopris and email notifications
allowed/delegated to authorized
personnel for completeness of
registration and for future processing of
coop amendments, branches and
satellites.
Step 2. Application Type
Once logged in, a dashboard will appear and the client must click “New
Registration” button.
Step 3. Cooperative Reservation form
The client shall fill-up the required information  in the Cooperative
Reservation form.

1. Major Industry Classification and its Subclass (select from dropdown list)
2. Proposed Cooperative Name
3. Common Bond of Membership (select from dropdown list)
4. Area of Operation (select from dropdown list)
5. Composition of Members (select from dropdown list)
6. Proposed Cooperative Address

The client must proceed to the next step (Step 4), otherwise, the reserved
cooperative name will expire after (4) four days and may be used again by
the public.
Step 4. Cooperative’s Information

1. The client must fill up the online cooperative information form to


generate  the Articles of Cooperation, By-Laws, Economic Survey
and Treasurer’s Affidavit.

Ensure that the names of Cooperators/Staff/Employees are in the PRS


Certificate and CETOS for proposed Transport Cooperative.
Step 4. Cooperative’s Information
1. The client must fill up the online cooperative information form to generate  the Articles of Cooperation,
By-Laws, Economic Survey and Treasurer’s Affidavit. 

2. Upload scanned copies of other required documents and submit by


clicking the “Submit” button.

3. The client must accomplish this step within thirty (30) days.

The client may regularly check the status of the application by logging in
the E-CoopRIS using his/her account.
Step 5. Processing, Review and Approval of
Application

The Authority shall process and review the applications on the basis of
the information submitted and documents uploaded in the E-CoopRIS
and in accordance with the Guidelines issued by the Authority. The client
will be notified of the result of the evaluation which may either be
approval, deferment or denial.

In case of deferment, the client shall comply with the findings within
fifteen (15) days from receipt of email by logging in to the system.
Step 6. Submission of Hard Copies of Documents
1. Once approved, the client shall print and submit the hard copies (all original)
of the required documents to the  Authority  within thirty (30) days from the
date of email notification.
a. ARTICLES OF COOPERATION should be NOTARIZED and its pages should be signed by all
cooperators (4 copies)
b. BY LAWS should be signed by all cooperators in the last page (4 copies)
c. ECONOMIC SURVEY should be notarized (4 copies)
d. SURETY BOND (1 original copy, 3 photocopies signed and certified by the Chairperson)
e. PRS CERTIFICATES and other certificates/documents required (4 copies)

2. Failure to submit the same shall be considered as an abandonment of the


application and thus, will be purged from the E-CoopRIS.

All documents submitted by the applicant shall be reviewed by the assigned CDA personnel to
confirm that the hard copies are identical to the soft copies previously filed online.

Any variance between the hard and soft copies submitted by the applicant to the Authority
shall result in the non-issuance of Certificate of Registration.
Step 7. Payment of Assessed Fees
Once the submitted documents are in order, the client will receive
payment advice through email. The client must log-in to the E-CoopRIS
account and click the payment button and order of payment will be
generated. The client shall choose the following payment method. 

a. Online Payment Through LANDBANK Link.BizPortal

The client shall fill up the required fields and submit. A payment  
confirmation slip will be generated.

In case of payment through LANDBANK, the client shall be  charged with the
following:

❑ Transaction fee of P10.00 per approved transaction  and 


❑ An additional P10.00 transaction fee will be charged by BancNet for the
use of its Internet Payment Gateway Facility.
Step 7. Payment of Assessed Fees
b. Payment Thru CDA Treasury
The client may login in the E-CoopRIS to view/print the order of payment. The client
then proceeds to the CDA Office Cashier to pay the assessed fees. The Cashier shall
issue Official Receipt.

Payment shall be made within ten (10) days from notice. Failure to pay within the period
will result to the automatic removal of the application from the system.

Acceptable payment should be made in:

❑ Cash;
❑ Postal money order or
❑ Manager’s Check
Step 8. Issuance of Certificate of Registration
The Authority shall issue Certificate of Registration upon confirmation in
case of online payment or issuance of official receipt if the payment is
through the CDA treasury.

Registration is successfully completed upon issuance of the Certificate of


Registration.
