FSP Form Filling Guidelines
FSP form should be filled in BLOCK letters in black ball
point pen.
FSP has to sign on each and every page of FSP form.
Latest Color Photograph should be pasted in Box in proper
alignment & Signature should be done in prescribed Box
only.
FSP Form Filling Guidelines
For DL ‘s FSPs– DL’s CG Code to be mentioned in the field “UM/BP
Code” and DL’s advisor code in “UM/BP Application No. “ field
If DL’s CG Code is not generated –
1. Dummy DL Code to be entered in the field “UM/BP Code” field
2. It is mandatory to enter Advisor Code in “UM/BP Application No. “ field of DL
whose CG code needs to be generated
Personal Information on FSP form
FSP’s e-mail id & mobile number are mandatory fields while
filling the FSP form as they are mandate for Online training and also help
FSPs in future communication purpose.

For FSP from rural area following fields are mandatory
Rural Code, District Name,Tehsil & Village Name
Process to get above details:
1.Go to Intranet
2.Click on Applications
3.Click on Location Classified Module
Personal Information
After clicking on Location Classified Module - Select State,
District, Tehsil & Village to get Rural Code.
If the Rural code is not available, then please note it is not a Rural Area.

Fill Rural Code, District, Tehsil & Village in below
mentioned fields.
Personal Information on FSP application form
On the FSP form refer point #12
Fill Basic Qualification, Board Name, Roll Number, Year of Passing
No supporting documents are required if you are submitting the Graduation proof.
Training & Exam Details
Exam Center & Exam Language: Mention correctly as
NSEIT- (Centre Name) - same can be checked in AMS for reference
While UM profiles FSP on navigator, he gets a drop down to select the centre
name.
Precaution has to be taken while selecting the mode of language.
Re-Login of FSP due to TCC expired of Old FSP
For existing valid URN with ICICI Prudential Life Insurance
please mention previous URN No & Previous FSP No for
tracking history or details of FSP.
Validity of URN is one Year.
Training & Exam Details
For TCC Expiry Case having valid URN with ICICI
Prudential Life Insurance please select Retraining as Yes while
profiling FSP in Navigator.
License Composite request
Composite conversion case - In case an advisor who is
currently licensed with General Insurance Co. and also want to be
associated with ICICI Prudential Life Insurance Company Ltd., then copy
of valid General Insurance License is required.
License Transfer Process
LicenseTransfer Case - In case of an existing Life insurance advisor of
some other insurance company, who is interested to be associated with
ICICI Prudential Life Insurance Company Ltd.
Mandatory document required
Copy of NOC on the company letterhead .
Copy of valid life License.
DOPS Microsite
FSP FOS related details updated by CDOPS team will be now
available on DOPS Microsite on daily basis (
http://10.16.218.153:90/DOPSNEW/index.htm)
Code Generated Report (FOS codes created by EOD)
Code Creation Pending with Remarks (FOS codes creation on hold due to de-
dupe / discrepancy)
Composite cases (All Composite request placed on IRDA portal by CDOPS
team and status of the same)
Transfer cases (All License transfer request placed on IRDA portal by CDOPS
team and status of the same is available.)
Welcome kit Dispatch Details (License and welcome kit of advisors and
dispatch status)
IRDA Status Report (All FSPs that are registered on portal are updated on daily
basis. )
Upload Error Report (All Portal Rejected FSPs)
Re-exam Scheduling Status (All Re-exam fee paid FSP status with details are
updated on daily basis).
