The following information shall be available during the HAZOP study : Process flow diagrams Piping and instrumentation diagrams (P&IDs) Layout diagrams Material safety data sheets Provisional operating instructions Heat and material balances Equipment data sheets Start-up and emergency shut down procedures EXAMINATION PHASE:- Examination phase can be described graphically as follows: HAZOP Guide word:- GUIDE WORD MEANING

DOCUMENTATION AND FOLLOW UP PHASE:- Documentation and Follow up Phase: Templates are used for recording the observations of the HAZOP analysis. The template can be modified by the team members when the team members when required to do so due to certain regulatory requirements ,policies or other factor. APPLICATION:- Applications of HAZOP analysis can be found mostly in Chemical, Pharmaceutical, Oil & Gas, Nuclear and other process industries for initiating new projects/process, modification and periodic review of the operations.
