• WHAT IS A HAZARD?
A HAZARD IS A CONDITION THAT IS A SOURCE OF RISK OR
DANGER TO A PERSON DOING A JOB.

• Many plant safety, and health, environmental, operating and


maintenance procedures detail potential hazards associated with
using a particular procedure. All potential hazards are not readily
apparent; an example might be the condition of brakes on a
bicycle one is about to use.

• Today we will discuss some hazards that may or may not be
apparent, to the person doing the job.
• Personnel that participate in such things as writing Safe
Work Plans, Safety Audits, Inspections and Housekeeping,
audits and reviews such as JSA’s must become familiar
with:
• RECOGNIZING HAZARDS WHEN THEY SEE THEM.

• Don’t accept it……………Fix it!


HAZARD RECOGNITION

• This FOLLOWING list contains, by no means, all


possible hazards but will serve as a guide for
those who wish to participate in the previously
listed activities.

• General Observations: (Use your senses sight, touch,,


Hear, Smell, Taste)
• Permits
– Properly filled out and signed and dated.
– Copy in the field at job site
– Proper PPE as specified on the permit
– Air monitoring results recorded and legible
– Hot work (if applicable) designated

• Unusual odors
– HYDROCARBON
– H2S
• Vibration

• Noise

• Temperature

• Observe a leak, drip, or equipment movement, smoke, ice formation

• TOOLS AND EQUIPMENT:


– Check all Safety equipment
– Face shields
– Respirators
– Respiratory equipment
– Safety showers/eyewash stations
– Head, eye, hand, and foot protection
– Fire hoses, fire extinguishers
– Slicker suits
– Air monitoring equipment
– Valves below process safety valves
– Bolts in flanges ( short bolts)

training of people on the scaffold

maintenance, electric cords and
hand tools.

• Locks from all skills applied and tagged
• Lockout procedure being followed

• Motorized equipment( cranes, pickers, forklifts, manlifts and
etc.)
• Capacity visibly marked
• Inspection current
• Operator trained
• Alarms and gauges and lights working

• Area Hazards:
– Housekeeping
– Slips/trips and Falls
– Barricades
– Yellow lined areas
– Algae growth treated
– Fall protection in use? 100% Tie off, harness in use
– Ladders in good working order.
– Pinch points ( sharp points, tight spaces)
– Proper PPE for the job being performed
– Access/Egress clear, emergency exits lit, emergency evacuation
points clearly understood and procedure for evacuation clearly
defined

• Is testing, commissioning, decommissioning, hydrostatic,
and pneumatic testing by procedure
• Is equipment being worked properly isolated and tagged
• Are all permit requirements being met

• Exposures:
• Are materials, supplies and wastes being handled properly
• Potential exposures to:
Chemicals, toxics
Heat or Cold
Flow or Pressure
Electricity

HAZARD RECOGNITION
• Chemical exposures:
Chemical hazards
Are workers or employees complaining of health problems
Have employees or workers viewed the MSDS’s
for the chemicals that may be present
Is air or personnel monitoring indicated
Is proper PPE available
• Health:
Restrictions for workers
Is medical surveillance needed for workers on this
job

HAZARD RECOGNITION
• Energized sources:
Live electrical equipment/ locked out?
Grounding in place
Static electricity an issue (vacuum truck operations, Fill rates
on tanks)
Is all electrical equipment intrinsically safe

• Environmental:
• Bleeders capped or plugged (tagged if open for de-pressure)
• Is heat exposure a possible problem on this job
• Is lighting adequate
• Noise is this a high noise job

• Excavations:
• Is excavation deeper than 4 feet
• Is excavation properly shored or braced
• Is a competent person available for required inspections daily
• Have underground hazards been identified
• Have the hazards of water or chemical accumulation been
identified
• Has the stability of adjacent structures been identified
• Are proper barricades in place
• If required has confined space permit been issued\
