▪ Ensure transfusion record is completed


▪ If NO reaction discard blood tubing and bag into biohazard
▪ Return copy of transfusion record to lab.
▪ If reaction return blood bag/tubing/tag to lab for analysis.
▪ Return copy of transfusion record – and reaction record to lab
▪ Continue to monitor according to facility policy
▪ Administer post transfusion medication

▪ If ABO and RH unknown type O-
Negative or O-Positive
unmatched blood is
administered.
▪ O-Neg given to female children
