MORTALITY:01
Female pt 50yr old (k/c DM,HTN) presented on 20/01/22 , with
compliant of chest pain, dyspnoea on/off on exertion for 3 months
ECG and Trop was suggestive of NSTEMI, and she got admitted
for Cor-Angio. Angiography showed TVCAD with LAD,LCX &
RCA having 99% stenosis. Pt was advised CABG but the attendants
of the pt refused for CABG ,later on during admission pt had
recurrent MI and went into pulmonary edema suggestive of HF with
KILIP class IV bed side echo showed LVEF : 35% with acute
severe MR. During the stay pt was hemodynamically unstable and
was on dual ionotropic support.
As the pt was not willing for CABG helplessly attempt for PCI was
made with high risk consent .Unfortunately she went into asystole
during the procedure, measures were taken to safe the pt but she
couldn’t survive despite resuscitation on 24-01-22.

72yr female pt (k/c IHD,DM) was admitted in CCU via ER on
19/01/22 with complaint of SOB for 10 days
O/E pt had B/L fine basal crepts and generalized body swelling was
found. ECG showed LBBB with raised troponin level.
Pt was managed on line of decompensated ICMP EF:20% but pt was
unable to maintain vitals therefore dual ionotropic support was
started, due to low SpO2 and delirious state of pt she was shifted to
MICU with high risk counselling of the attendants. Pt expired in
MICU on 28-01-22 with cardiogenic shock .
