Definition

A system that controls the removal and re-introduction of


energy to equipment and facilities being serviced.

Lock-out/Tag-out is also a method of keeping equipment from


being set in motion and endangering workers
Hazardous Energy Control or Lock-out

“ Lockout” means to physically neutralize all energies in a piece


of equipment before beginning any maintenance or repair.
Lockout generally involves:

 Stopping all energy flows( for example by turning off switches,


or valves on supply lines, circuit breakers or other energy-
isolation mechanisms are placed in the safe or off position)
 Locking switches and valves

 Securing a machine, device, or power transmission line in a de-


energized state ( for example, by applying blocks or blind, or
bleeding hydraulic or pneumatic pressure from line)
Lockout is an integral part of PTWS

 Ensures proper isolation of all energy sources on equipment.

 Controls de-isolation to ensure all work is done before startup.

 Controls the re-instatement of equipment and facilities.

 Minimizes risk of personnel being subjected to potential


energy.

 Eliminates near misses and incidents associated with work at


facilities
Importance of Lockout
To avoid:

 Electrocution ( contact with line circuits).

 Cuts, burses crushing,amputations,death resulting from:


- Strangulation with belt, chains, conveyers
rollers, shaft, impellers
- Drawing in liquid in vat or tank

 Burns ( contact with hot part e.g.: equipment such as furnace)

 Fires and explosions ( gases or liquids released from pipeline


When should you Lockout and/or Tagout

Lock-out/Tag-out is necessary whenever you are working


on and around an equipment and you could be injured by:

 Unexpected start-up of the equipment

 The release of stored energy


Lockout / Tagout Procedure
1. Preparation for shutdown

Before you turn off any equipment in order to lock or tag it


out, you must know:
 The types and amounts of energy that powers it.

 The hazards of the energy

 How the energy can be controlled

2. Equipment shut down & Isolation
 Make sure the switch you are about to pull is the right one.

 Push the button or turn the selector switch to the off position.

 Move the operating lever to the “OFF” position.

 Do not apply excessive force to the operating handle

 Operate all energy-isolation devices to ensure that the equipment is isolated from its
energy sources.

 Be sure to isolate all energy sources, secondary power supplies as well as the main one

3. Application of LO/TO

 All energy-isolating devices are to be locked and tagged.

 Use a lockout device if your lock cannot be placed directly on the


energy control.

 Every employee in the work crew must attach his personal key

 Fill tags out completely (Name,Date,Trade,and Reason )

4. Control of Stored Energy

 Inspect the system to make sure all parts have stopped moving.

 Check for voltage and install ground wire ( if applicable).

 Release the tension on spring, or block the movement of


spring-driven parts.

5. Verification
 Make sure all danger areas are clear of personnel.

 Verify that the main disconnecting switch or circuit breaker can’t be


moved to the on position.

 Press all start buttons and other activating controls on the


equipment itself.
Outcome

 Do not work on any equipment unless you have been trained and competency
tested on lock-out/tag-out procedures for your area and for the equipment.

 Do not work on any equipment until you have checked and verified that it is at
zero energy.

 Only trained professional electricians may work on electrical systems. They


must ensure that electrical circuits are de-energized before repairs are made.

 For an unprofessional technicians ask for helps regarding your task while
performing the job.
Summary

Lock-out/tag-out is the most important part of our Permit to


Work System.

This system though cumbersome keeps us safe and it does


work.

Be your brothers' keeper. All it takes is 1 shortcut and


someone gets hurt or killed.

Use Lock-out/Tag-out to ensure a work place free of incidents


and near misses.
