Functional Risk Categories

FR1 – 98% replaces Very High Risk (VHR) for audit score and frequency
FR2 – 95% replaces High Risk (HR) for audit score and frequency
FR3 – 90% new risk category giving flexibility for a more realistic expectation of standards in long
term wards and treatment centres
FR4 – 85% replaces Significant Risk (SR) for audit score and frequency
FR5 – 80% new risk category giving flexibility for a more realistic expectation of standards in high
use areas such as receptions and prayer rooms
FR6 – replaces Low Risk (LR) for audit score and frequency

FRB – Blended is a new risk category which sits outside the 6 functional risk ratings, the target
percentage for a blended approach is based on the combined targets for rooms in each functional
area.

FR4 85% and above Every 3 months

FR5 80% and above Every 6 months

Audit Scores will be represented in 2 ways, percentage score for internal verification and reporting
and a star rating score for external verification and the public.

Percentage scores will still be split by the responsible group, i.e. cleaning, nursing and Estates in
order to understand any gaps in performance
