LAPAROSCOPIC SURGERY

De Guzman, Princess M. Group 2 BSN 4-1

Laparoscopy
Also Called: Minimally Invasive Surgery (MIS) Band-aid Surgery Keyhole Surgery

Definition
is a modern surgical technique in which operations in

the abdomen are performed through small incisions (usually 0.51.5 cm) as compared to the larger incisions needed in laparotomy.

Continuation
Keyhole surgery uses images displayed on TV

monitors for magnification of the surgical elements.

Laparoscopic surgery includes operations within

the abdominal or pelvic cavities, whereas keyhole surgery performed on the thoracic or chest cavity is called thoracoscopic surgery. Laparoscopic and thoracoscopic surgery belong to the broader field of endoscopy.

Continuation
There are a number of advantages to the patient

with laparoscopic surgery versus an open procedure. These include reduced pain due to smaller incisions and hemorrhaging, and shorter recovery time.

Purpose of Diagnostic Pelvic Laparoscopy

Used to examine internal organs of the pelvis in

able to indentify a specific problem.

Reasons for Pelvic Laparoscopy

Pelvic pain or mass
Accumulation of fluid Difficulty becoming pregnant

Instrument Line

Flushport designed to pass a high volume of liquid

through the shaft to more effectively remove gross debris prior to sterilization

Nursing Care prior the procedure:

IV pain medication and sedative
General Anesthesia Breathing Tube Catheter placed into the bladder

Step by Step Procedure

A sharp instrument called TROCHAR, would be

inserted making a small half inch opening just below the navel.
Carbon Dioxide Gas would be then connected

and pumped to this umbilical port to pop up the abdomen, so that contents would be viewed more easily.

Step by Step Procedure

The surgeon would insert the laparoscope on the

umbilical port.
Images from its camera would be viewed on the

monitor.
The surgeon would carefully examine organs and

tissues looking for symptoms that can explain your condition.

Step by Step Procedure

He/ She may create other opening where other

instruments can be slit. The additional instruments may be used to loop organs out of the way for better visualization of the underlying tissues or to perform other operative procedures such as removing scar tissues, taking biopsies or draining abnormal fluid. When the laparoscope and other instruments are removed, a special gas bulb will be left briefly to allow carbon dioxide to escape the abdomen.

Step by Step Procedure

The key hole incision would be then closed with

just a few sutures or staples. And then covered up with bandages.

Nursing Care after the Surgery:

Breathing tube will be removed.
Patient would be monitored on the recovery area. Catheter will be removed. Pain medication will be given. IV line will be removed when you can able to

drink fluid orally.
