and Diagnosis?

07 CONCLUSION:
What is DIKW?
 DIKW is a pyramid that provides a conceptual representation of
governance and information management systems with reference
to Maslow's hierarchy of needs. Called by several names: the
Information Pyramid, Pyramid of Knowledge, Pyramid of
Wisdom and, less generally, Information Hierarchy or Hierarchy
Knowledge.
What is DIKW?
DATA:

 Data are elements used and not interpreted only by using data
from observations.
What is DIKW?
INFORMATION:

 Is a collection of data that contains meaning. Data will be stored

permanently as a result of patient health documentation.
What is DIKW?
KNOWLEDGE:

 The Inter-relationship between the information and data that has

been studied. The results of the formula can be described or
treated using a computer. In addition, knowledge can also be
interpreted as a formula or as a set of rules.

 Putting all together in one piece.

What is DIKW?
WISDOM:

 Wisdom is a capacity for judgment involving ethics and aesthetic

values.
What is Nursing Process?
 Ida Jean Orlando started the nursing process that still guides
nursing care today. Defined as a systematic approach to care
using the fundamental principles of critical thinking, client-
centered approaches to treatment, goal-oriented tasks, evidence-
based practice (EDP) recommendations, and nursing intuition.
Holistic and scientific postulates are integrated to provide the
basis for compassionate, quality-based care.
The Nursing Process has 5 parts:

 A – Assessment

 D - Diagnosis

 P - Planning

 I - Implementation

 E - Evaluation
Similarities of DIKW and Nursing
Proess:
 Both serve as a framework to enhance nurses’ skills in making sound clinical
decisions to ensure client-centered and organized approach to patient care.

 Useful tools that assist nurses to analyze and process data in a step-by-step
manner which helps in developing a more accurate and complete information.

 Stimulate and enrich critical thinking skills and strategic planning

 Assist in carrying out data-based care management, skills and knowledge for
subsequent actions.

 Both work to collect all the results obtained in order to be organized, relevant,
structured and well data.
Similarities of Data and Assessment:

 Data and assessment are products of observation and initial

findings.

 In a clinical setting, they are stored within patient records and are
used as a basis for further reasoning, collection and discussion.

 Both are characterized as either subjective or objective.

Example 1:
 40
 Female
 Experiences pain in her ankle
 Temp: 37.5°c
 PR: 105
 RR: 22
 BP: 116/70 mmHg
Example 2:
 20
 Female
 College of Nursing Student
 Always sleeps late
 Does not have an exact sleep pattern/routine
Similarities of Information and
Diagnosis:
 A way used to describe a group of feelings, experiences or symptoms.

 Usually based on a person’s medical history and an assessment of the

person seeking medical help.

 Both served as a basis for problem solving and reasoning.

Example 1:
 A 40-year-old woman complaints of pain in her ankle. She suffered
trauma to her ankle which she fell down in a hole, her examination
reveals a fracture, and she will need casting. When assessed, the
woman’s body temperature is 37.5°c, PR was 105 beats per minute, RR
was 22 cycles per minute and BP was 116/70 mmHg.
Example 2:
 The 20-year-old female reports being weak and reported that she does
not have an exact sleeping routine due to overloading school activities,
hence she always sleep late at night.
Similarities of Knowledge and Planning:

 Arises from the interrelationship of information and/or diagnosis.

 Awareness and understanding of information and/or diagnosis that has

been obtained.

 Deals with problem solving and reasoning.

Example 1:

 Create a care plan to manage patient’s pain and promote stability

at the fractured site.
Example 2:

 Formulation of care plan to improve patient’s sleeping pattern

along with time-management tips.
Similarities of Wisdom & Intervention
and Evaluation:

 Determining the patient’s need of care from the given data.

 Application of knowledge to actions in delivering delegated care.

 Determination of the patient's responses to the nursing interventions.

 Compare data related to outcomes and the extent to which the outcomes
have been achieved.
Example 1:

 The nurse instructs patient to maintain bed rest or limb rest as

indicated. The nurse also provided health teaching about proper
body mechanics to the patient that provides stability, reducing the
possibility of disturbing alignment and muscle spasms, which
enhances healing. Then, after 8hrs of intervention, the client
demonstrated proper body mech and verbalized decreased pain.
Example 2:
 The woman was educated about proper time management and
use of soporifics such as milk before going to bed in order to
induce sleep. Patient was also taught about nursing aids such as
comfortable sleeping position, and relaxation techniques to
stimulate sleep. After the nursing interventions, the patient was
able to report optimal balance of sleep with improved sleeping
patter and less agitation about school activity/ requirements.
Complete Example
1:
DATA:
 Age: 40
 Gender: Female
 Experiences pain in her ankle
 VITALS: Temp: 37.5°c PR: 105 RR: 22 BP: 116/70 mmHg

INFORMATION:
 A 40-year-old woman complains of pain in her ankle because she fell down in a manhole, and her examination
reveals a fracture. When assessed, the woman’s body temperature is 37.5°c, PR was 105 beats per minute, RR
was 22 cycles per minute and BP was 116/70 mmHg.

KNOWLEDGE:
 The woman suffers from acute pain related to ankle fracture as evidenced by x-ray results of fracture and
verbalization of pain in her ankle. The nurse then created a care plan to manage patients' pain and promote
stability at the fractured site.

WISDOM:
 The nurse instructs patient to maintain bed rest or limb rest as indicated. The nurse also provided health
teaching about proper body mechanics to the patient that provides stability, reducing the possibility of
disturbing alignment and muscle spasms, which enhances healing. Then, after 8hrs of intervention, the client
demonstrated proper body mech and verbalized decreased pain.
Complete Example
2:
DATA:
 Age: 20 College of Nursing Student
 Gender: Female
 Always sleeps late
 Does not have an exact sleep pattern/ routine.

INFORMATION:
 The 20 y/o female reports being weak and reported that she does not have an exact sleeping routine due to
overloading school activities, hence she always sleep late at night.

KNOWLEDGE:
 The woman has disturbed sleep pattern related to non-restorative sleep patter as evidenced by the verbalization
of not having an exact sleep time due to overloading school activities. The nurse was able to formulate a care
plan to improve the sleeping pattern of the patient along with ways/ tips on how to manage her time wisely.

WISDOM:
 The woman was educated about proper time management and use of soporifics such as milk before going to
bed in order to induce sleep. Patient was also taught about nursing aids such as comfortable sleeping position,
and relaxation techniques to stimulate sleep. After the nursing interventions, the patient was able to report
optimal balance of sleep with improved sleeping patter and less agitation about school activity/ requirements .
Conclusion:
In this presentation, we were able to define the similarities between DIKW and the Nursing
Process. We also gave examples to have a clearer understanding of this topic.

In conclusion, DIKW and The Nursing Process are similar because they are both considered
systematic and cyclical processes. Both have their building blocks in order to achieve a specific
goal. Hence, these so-called building blocks are a group of collected data categorized and
analyzed to form the inter-relationship of information to each other that will eventually lead to
achieving the goal.

A more solid understanding of the similarities between DIKW and the Nursing Process allows
you to have an idea about an assertive problem-solving approach to identifying and solving the
existing problems.
