Definitions
• Asymmetrical Load: An object with an unknown center of gravity due to its shape or
composition.

• Boom Angle: The angle above or below horizontal of the longitudinal axis of the base of the
boom section.

• Cargo Carrying Unit (CCU): Load Carriers (Container/ Basket/ Half Height/ Tank/ Rack)
• Preventive Maintenance System (PMS)
• Competent Person: A person who has been suitably trained to perform a task to the required
standard, i.e., has completed the relevant training (such as crane operator school, rigger
school, etc.) and has the applicable certifications required for the operations.
• Deck Crane: A slewing crane for general use mounted on an installation and which is used to
handle loads to and from boats.

Definitions
• Lifting Appliance: Mechanical support devices that do the lifting.
• Lifting Appliance Operator: An individual who is qualified to operate mechanical support
devices that do the lifting other than the deck crane.
• Lifting Accessories: Below the hook rigging or equipment that connects items being lifted.
• Load Rating: The crane ratings in pounds (kilograms) established by the manufacturer.
• Nominal Capacity: The actual hook load that the crane is designed to lift in each operating
condition. Nominal capacity for an offshore crane is stated in the crane’s load chart for
different significant wave heights.
• Personnel Lifting: Operations where a lifting appliance transfers personnel to, from, and
around offshore & Onshore rigs by suitable certified equipment and personnel carriers.
• Safe Working Load: The maximum load the sling or lifting component is certified to
withstand under normal use and in each configuration.
• Elevated Working Platform: A mechanically operated work platform that moves vertically in
guides.

Definitions
• Heavy Lift: Any load weighing twenty (20) Metric ton or greater.
• Fixed Attachment Point for Lifting: A place for temporary attachment of lifting equipment.
• Inspection: A visual control of lifting equipment for defects and a check of operating controls,
limit switches, and indicators.
• Banksman: A competent person who is responsible for directing movement of the crane and
the load

• Slinger: The individual responsible for ensuring that all slings, chains, and hooks are properly
secured on the load and that the load is prepared to travel.

Color Codes
• The lifting gears inspection (LGI) shall be conducted by certified 3 rd party every 6 months with
painting the accepted lifting gears by the current color code.

July 2023 to 31 December 2023

January 2024 to 30 June 2024

July 2024 to 31 December 2024

January 2025 to 30 June 2025

• The rejected lifting gears shall take Red color.

Stop Work Authority (SWA)/Time Out for Safety (TOFS)
SWA/TOFS shall be applied to lifting operations
when:
• Communication (visual or radio) with the Operator
is interrupted.
• Anyone touches, or attempts to touch, a
suspended load or rigging under tension.
• Any person is in the travel path/Line of Fire of the
load.
• Anyone exhibits the “Emergency Stop” signal.
• The lifting appliance presents a technical fault or
upon any visual or audible alarm.

Blind Lifts
• A “blind lift” is Lifts where the lifting appliance operator does not have a direct view of the Banksman or the
Banksman does not have a direct view of the load. For any blind lift, the signaler / Banksman must be in radio
contact with the lifting appliance operator and must give constant verbal communication. If radio communication
stops, then the lift stops until communication can be re-established.
• If the appliance operator has a direct line of sight with the Banksman / signaler, and the Banksman / signaler has
a clear line of sight with the load, this is not a blind lift. However, if multiple banksman /signalers are required due
to the complexity of the operation and the signals must be passed through multiple banksman / signalers then the
lift would be classified as a blind lift.
Note: The use of closed-circuit television monitoring (including boom tip cameras) is not considered to be an acceptable
substitute for personnel with direct view of the load or landing area
• No blind lift is allowed for personal transfer during day if the crane operator doesn’t have visual eye contact with
the transfer basket.

Control of lifted loads
• All loads shall be controlled by a dedicated tagline, or by an approved
push/pull pole from Hands Free Tools as per ADES Hands Free Catalog.
• When performing lifting operations to or from a vessel, the vessel master
should be aware of the planned use of taglines.
• All taglines for general cargo-carrying units and tubulars shall be a minimum of
15 feet (4.5m) long.
• The length of taglines within confined areas such as the drill floor and
machinery spaces shall be specifically considered and documented in the JSA.
• All cargo-carrying units’ dimensions in excess of 10 feet x 8 feet (3m x 2.4m)
shall have a minimum of two taglines attached.
• No person shall allow a tagline to run through their hands while a load is being
lifted.
• When using a tagline, the slinger shall ensure that the tagline is never
wrapped Around any body part.
• Never use taglines with knots or frayed ends.
• Where the tagline is under a load, use an approved push/pull pole to retrieve
the tagline.

Lifting Operation wind speed Limitations

• For lifts with cranes, lifting operations shall cease


when the continuous wind speed exceeds 40 knots
or as per the crane manufacturer’s instructions
(whichever is less).

• Lifts conducted in continuous wind speeds greater


than 30 knots require a Task Risk Assessment.

• All crane lifting operations shall be undertaken by a minimum of Three certified
personnel: the Operator, the Banksman, and the Slinger.

• Limit switches shall not be used routinely as a mechanism to stop a crane boom or
hoist. Activation of a limit switch for any purpose other than test shall be considered a
Near Miss and recorded in ADES reporting platform system.

• Lifting operations over wireline, coil tubing, or pressurized test lines is prohibited.

• No snatch lift is allowed for any lifting loads from supply boat to the offshore Rigs.

• All crane/winch cables shall be changed out every two years, Unless OEM dictates
different change out frequency for the crane cable. In case damage is found, in which
case they shall be changed out immediately.

• Pennant line cables need to be changed every five years, unless damage is found, in which case they shall be changed
out immediately.

• The air supply lines to all air hoists shall be fitted with a pressure regulator of a size to deliver sufficient volume for
the hoist to operate within manufacturer's recommended parameters, and adjusted to either the manufacturer's
recommended pressure limit, or 90 psi, whichever is less. Such air pressure regulators shall also have either a lock or a
tamper-proof feature to discourage or prevent the adjustment of the regulator to discharge pressures above the
requirements of this standard.

• Crane windows shall be kept clean and free from defects in the operators’ line of sight that could affect visibility.

• The Use of 3rd Party Transit Slings for any other purpose that the transport of tubulars/etc. to the rig is prohibited. All
3rd Party Transit Slings are to be removed once arriving onboard and segregated in a dedicated storage area to keep
from mixing with ADES Lifting gears.

• No blind lift is allowed for personal transfer during day if the crane operator doesn’t have direct eye vision of the
banksman and good weather visibility is a must.
NOTE: The use of closed-circuit television monitoring (including boom tip cameras) is not considered to be an acceptable substitute for
personnel with direct view of the load or landing area.

• All Taglines for general cargo-carrying units and tubulars shall be a minimum of 15 feet (4.5m) long.

• All cargo-carrying units’ dimensions in excess of 10 feet x 8 feet (3m x 2.4m) shall have a minimum of two taglines
attached.

• Where the tagline is under a load, use an approved push/pull pole to retrieve the tagline.

• Lifting of Metal Plate


All lift of plate metal must be by an approved mechanism:
a. Factory Pad Eye – with load lest and MPI of weld by an accredited 3rd Party as per client requirement.
b. Drilled Hole – Hole is to be on the centerline of the plate and 2” from the edge. Hole shall be of adequate size to allow
a shackle of sufficient load rating to be used.

• Dynamic load charts shall be used for all lifting operations involving vessels. The crane operator shall not lift loads
that exceed the crane’s nominal capacity according to the dynamic load chart.

• All crane movements must be stopped during Helicopter Operations.

• When helicopter is expected:


o Complete the lift operation or lay down the load.
o Boom away from the heliport ensuring the boom is out of the flight path or cradle the boom.

• When lifting any tubulars, slings shall be double wrapped and choked. Slings on bundles of tubulars shall be secured to
prevent uncontrolled loosening when the load is landed as and where required. Slings should be choked in the same
direction and have the same SWL and size for each lift.

• Methods of transporting Super sacks:
o Preferred method is Load Carriers (CCU or half height basket)
o Secondary method, Spreader Bar (Offshore) or Big Bag Ring (Onshore), is to be used as per OEM design. (Rig
manufactured/Homemade bar is not allowed.)
o Alternate method for Offshore, Four-legged wire rope slings measuring at least 2 meters with affixed self-closing
hooks.

• When the bag is suspended the loops should be vertical without any twists or knots.

• Pallets Lifting Requirements:


o Pallets must be lifted only by forklift, pallet truck, or certified pallet lifting devices.
o The preferred option is Cargo Carrying Unit (CCU)
o Never lift double pallets with Pallet Forks
NOTE: Pallets must never be lifted simply by using slings.

Personnel Lifting Procedures
• The wind speed shall not exceed 25 knots during any type of personnel lifting.

• Personnel transfers at night shall only take place after the acknowledgment of the Rig
Manager through formal, written approval in accordance with comprehensive risk
assessment.

• Night personnel transfers shall be implemented after the formal Rig Manager approval and
shall be directly supervised by the OIM and the Barge Engineer.

• No blind lift is allowed for personal transfer during day or night, if crane operator doesn’t
have direct eye vision of the basket and good weather visibility is a must.

• Banksman has the responsibility to inspect the Personnel transfer equipment before any
use and all lift requirements.

• The OIM/PIC is to approve all personnel transfers, while the radio operator is responsible for
creating a log of all personnel transfers.

Personnel Lifting Procedures
• Standby vessel, or an alternative means of rescue is on standby.

• For transfer of personnel to/from a vessel by personnel transfer equipment / work basket,
then a minimum of 3 personnel is required for the operation. Crane Operator, Banksman,
Tagline Operator. The Banksman is responsible for verifying personnel transferring to the
Vessel are wearing their work vest / Personal Flotation Devices (PFD) before the transfer
begins. This verification must be documented on the Lift Plan.

• Rig supplies shall not be transported by personnel basket at any time.

• The maximum wave or swell height at which the transfer of passengers offshore can take
place shall not be greater than 1.5m.

• Two pieces of personnel transfer equipment shall be on board at all times and ready to use
any time, for any scenario (Stretchers, Transfer Baskets.)

Work Baskets

• Personnel shall never climb on the rails or edge of the work


basket and shall never try to overreach.

• Personnel shall always keep arms and hands within the confines
of basket structure whenever that basket is being moved.

• Refer to manufacturer’s instructions for specific weather


operating limits.

Working Over Water

• Working over water shall not take place unless there is


sufficient light to perform an open sea rescue.

• Working over water shall not take place unless a standby vessel
is on close standby, or an alternative means of rescue is on
standby, i.e., MOB boat or rescue craft.

• A work vest shall be worn while working over water. Work vests
shall comply with regional regulations.

• If working from a basket over water, or where there is a risk of


falling into the water.

Lifting Equipment Procedure
• General Examination Requirements:
o SWL/WLL. Check that the SWL is adequate for the load.
o Unique identification number
o Check that the color coding is current and that the sling has a plant/ID mark.
o Manufacture identification
o Date of inspection / test.

• Examine the entire body and check for:


o Wear
o Stretch
o Corrosion
o Abrasion
o Mechanical Damage
o Internal deterioration
o Deformation

Quarantine Process
o When there is a noticeable defect
o Color code not changed
o Inspection certificate is not valid

• When a suitable quantity of quarantined equipment has been


amassed, it should be sent in for service/repair, with a manifest,
to be repaired / re-certified or disposed of.

• The BE (Offshore)/RSO (Onshore) should update the lifting gear


register to show the item has been quarantined and the reason
for doing so.
