Interviewing a patient for collection of data related to medical
history is known as medical history interview. Patient medication history interview is a practice associated with collection and recording of information by reviewing of patient related to his /her past and present medication used. It provides the valuable information on the patient’s allergic tendency, patient compliance and self-medications. GOALS AND NEEDS Main goal is to acquire the information on aspects of drug used by patients. So such information may help pharmacist for improvement in overall care of patient. The collection information can be helpful to achieve the following goals:
1. Helps to investigate the medication discrepancies by comparing the
medication profile with the medication administration record. 2. Other staff verify the collected history of patient and can provide additional information wherever require appropriately. 3. Helps to documents the allergies and drug related adverse reactions. 4. Helps to evaluate rational for prescribing of drugs. 5. Helps to assess indication of drug abuse. 6. Helps to check the drug interactions. 7. Helps to access drug administration techniques. 8. Helps to examine the requirement of assistance in medication. Importance of Accurate medication History: Helps in the prevention of prescription errors and subsequent risks to the patients. Helpful in finding of drug related pathology and any clinical signs due to results of drug therapy. Considering all accurate medication history information, it helps to establish better care plan for the patients. STAGES OF PATIENT MEDICATION HISTORY There are three stages involved while practicing for patient medication history. The objective and procedure at each stage are discussed below: Stage 1: Before taking of medication history. Objective: To create good relationship and build confidence in patients. Method: Following are different steps in this procedure: 1. Confirmation of patient identity. 2. Self-Introduction. 3. Reason of interrogation and total time required for interrogation. 4. Initiate in taking of medication history of patient. Stage 2: During Taking of history. Objective: To collect accurate information on the medication history. Method: Following is the step follow in this 5. Ask the question according to format given for collection of information on medication. Stage 3: After taking of history. Objective: Documentation and analysis of information. Method: Following are steps follow in this stage: 1. After completion of interview, pharmacist must express appreciation for providing this information. 2. Review and analyzes the past medical record. 3. Documentation of important medication history data of patient. TYPE OF INFORMATION NEED TO BE RECORDED The following information is commonly recorded while interviewing the patient for collection of information on medication: 1. Recently prescribed medicines. 2. Use of OTC medication. 3. Any vaccinations in past or recently. 4. Any social habit (e.g. Alcohol, smoking). 5. Any Use of traditional medicines.). 6. Any reactions and allergies occur due to use of medicine. 7. Medicines found to be ineffective. 8. Compliance with past treatment. 9.
