Definition
A benign neoplasm of fibroblasts with a propensity to differentiate into histiocytes.

Properties
1. The lesions can be aggressive
2. They represent true benign neoplasms of facultative fibroblasts
3. Facultative fibroblasts are mesenchymal cells that have the potential to
differentiate into both fibroblastic and histiocytic cells.
4. A malignant variant is recognized

Clinical features
1. Occurs anywhere in head and neck area
including soft tissue of oral cavity
2. Well demarcated
3. Movable
4. Firm or even indurated on palpation

Histopathology
1.Composed of elongated, spindle shaped fibroblasts
2.Fibroblasts elaborate mature collagen and large oval nuclei and cytoplasm
representing histiocytes
3.The tumor cells are arranged in fascicles and whorls
4.They are tightly interwoven with hypercellularity.
5.Mitosis figures are uncommon
6.Histiocytic cells show pleomorphic nuclei
7.Some of these cells are intensely stained cytoplasm
8.Multinucleated

Storiform pattern
The lesions are located superficially
within the tissue will frequently show
fibroblastic cellular arrays that
resemble a pinwheel
It is a characteristic of
fibrohistiocytic lesion.

Treatment
Some are self limiting otherwise
Wide local excision is the treatment of choice for large
lesions.
