Causative agents

Mode of spread
• Hematogenous spread ; vertebrae most common site and metaphysis of long
bone.
• Extension from contiguous site; previous surgery, wound, or poor vascularity
• Direct implantation ; traumatic injury or surgical contamination

Pathogenesis

• Affect mainly metaphyseal region of


long bone.
• Bacteria gain access through nutrient
arteries.

• Common reason for metaphyseal


osteomyelitis
• Hairpin bend vessels
• Pooling of blood due to increased
vascularity.
• Immature cells

Pathogenesis (cascade of events)

Infection begins at the metaphyseal end of marrow cavity and is filled with pus.

Tension in the marrow cavity rises due to pus and results in spread of infection.

Infection reaches the subperiosteal spaces subperiosteal abscesses are formed.

Combination of suppuration and impaired blood supply to cortical bone results


in erosion.
Thinning and infraction necrosis of the cortex i.e. called sequestrum.
New bone formation beneath periosteum encasing necrosed bone i.e. called
involucrum.
