diagnosed with COPD with hypertension

• To effectively manage both COPD and HTN by adhering to prescribed medication
regimens.
• Implementing lifestyle modifications such as dietary changes to reduce salt intake.
• Monitoring blood pressure and respiratory status regularly to minimize exacerbations.
• Improving overall quality of life
• Monitoring patient’s blood sugar continuously.
• Disease specific
• To improve the lung function and reduce exacerbations by implementing a
personalized treatment plan , which include smoking cessation , pulmonary
rehabilitation , and medicine adherence.
