Premedication In Anesthesia

• Goals of Premedication are:
1. Anxiolysis
2. Amnesia
3. Analgesia
4. Nausea and Aspiration prophylaxis
5. Reduction of Airway secretions
6. Perioperative stress reduction
7. Blocking of Autonomic reflexes
1.Sedation/Anxiolysis
a) Pre hospital visit and Parental presence
during induction.
b) Benzodiazepines: Midazolam, Lorazepam,
Diazepam
c) Barbiturates: Pentobarbital, Thiopental
Drugs given 1hr preoperatively
2. Analgesia
a) Opioids: Morphine, Sulfentanil, Fentanyl
b) NSAIDS: PCM, Ibuprufen, Ketorolac
3. Aspiration Prophylaxis
a) Gastric PH increasing drugs:
I. H2 receptor antagonists: Ranitidine
II. PPI: Omeprazole
III. Antacids: Sodium Citrate
b) Prokinetic drugs:
• Metoclopramide
4. Anticholinergics
• Are administered to:
a) Prevent Bradycardia and hypotension:
laryngoscope, intubation, surgical manipulation
b) Reduce airway Secretions
• Include: Glycopyrrolate, atropine,
scopolamine
5. Antiemetics.
• This target PONV
• They include
– 5-HT Antagonist: Ondansetron
– Metoclopramide
