My signature on this statement certies that I, ____________________________________________________ (parent- please print) And I, ________________________________________________ (dancer- please print) have read and agree to comply with the guidelines as set forth in this handbook for the 2012-2013 dance season as an active member of Above the Barre Dance Academy Dance Company. Upon entering into the company, I will make a commitment to my own, personal growth in the eld of dance by attending all scheduled rehearsals and company events and create a set of personal goals to achieve in each of my dance lessons. I will arrive on time, dressed appropriately and prepared to dance. To better support my team, I will seek assistance from other dancers of the company and return my services to fellow teammates when they are in need. I will project professionalism at all times. I am aware that if my child or I are to leave the company prior to the end of season or between payment collection dates, my payment will not be refunded nor prorated. I am also aware that any correct payments made to Above the Barre Dance Academy are nonrefundable. _________________________________________ date ________________________ (Dancer) I have read and agree to comply with the guidelines as set forth in this manual for the 2012-2013 dance season as a member of an/or parent of Above the Barre Dance Academy and further agree to pay tuition, convention/competition, rehearsal/choreography and costume fees by the method(s) offered to me in this handbook. I understand that any issues relating to Above the Barre Dance Academy that are not covered in this handbook, the website or the annual informational meeting are to be addressed to Chelsie Klein electronically. Discussion of matters related to Dance Company is prohibited in the studio at any time. Interrupting instructors during or between class/rehearsals is not permitted unless in the case of an emergency.
