I/We confirm that the information given in the service order form for Personal Collabrator/Audio Conferencing services is correct and complete in all respects and no information is withheld/hidden or given wrong, to get Personal Collabrator/Audio Conferencing service. I/We agree not to allow any transfer/sublet/handover/sale of Personal Collabrator/Audio Conferencing service to any person/s, parties. I/We agree not to allow any person to use this service for malicious objectives. If such incidence occurs, PTCL reserves the right to terminate service without any legal liabilities on its part. In case of business I/We hereby certify that we are authorized to sign this document on behalf of our organization and bind the organization for all the terms and conditions of this agreement.
